It includes printouts of non-medical and social Support services for seniors living in Richmond community. The handouts are easy to read and are meant for you to use and share with your patients, clients and their families. They are arranged according to the Table of Contents listed on the following pages. Richmond Public Health Falls Prevention Immunization 604-233-3150 Provides education and suggestions to help prevent falls at home. Will do home falls risk assessment. May include home modification recommendations, strengthening and balance instruction, identifying falls risk including nutrition, general health, and wellness. Provides flu immunization.

VCH SteadyFeet Balance & Mobility Program Minoru Seniors Centre: 604-238-8450 Steveston Community Centre: 604-238-8080 SteadyFeet is standardized, evidence and outcome supported balance and mobility program specially designed to be safe for older adults. Goals of the program are to improve functional fitness, build strength and confidence, and reduce fall risk to help older adults stay safe, active and independent. Classes are delivered by Registered SteadyFeet Instructors in community and senior centres within VCH. Provides multidisciplinary assessment and treatment for older adults who have diminishing cognitive function or complex psychiatric and medical needs. Family Support and education is also provided. Care is coordinated with primary care physicians, elder care facilities and other community programs. Services are provided on an outreach basis. Physician referral required. Multidisciplinary services include case management, outreach and rehabilitation programs. Physician referral required. Richmond Access Central Detox Referral Line 1-866-658-1221 Fax: 604-633-4231 A screening and referral telephone line that provides adults access to withdrawal management services and addiction housing.
