It includes printouts of non-medical and social Support services for seniors living in Richmond community.

2 The handouts are easy to read and are meant for you to use and share with your patients, clients and their families. They are arranged according to the Table of Contents listed on the following pages. For more comprehensive information on the services, please call the number provided and/or visit the website, if available. Direct home care services include: Nursing Physiotherapy Occupational therapy Nutrition services Case management Chronic Disease Management Services available to Home Health clients may include: Home Support Adult day program Assisted living Residential care Call Richmond Home Health to make a referral; eligibility criteria applies.

8 Richmond Community Respiratory Program 604-233-5610 Community Respiratory Program (Physiotherapist, Respiratory Therapist) provides care to individuals with lung conditions such as Asthma and COPD. Richmond Hospital 604-278-9711 Provides a wide range of health services. Richmond Public Health Falls Prevention Immunization 604-233-3150 Provides education and suggestions to help prevent falls at home. Will do home falls risk assessment. May include home modification recommendations, strengthening and balance instruction, identifying falls risk including nutrition, general health, and wellness.
