1 _____ PURPOSE OF THIS FORM _____ Lakeland Community College students receiving federal financial aid are required to meet Standards of Academic Progress (SAP). Students are considered to have an eligible SAP status if they: Maintain the required cumulative grade point average (GPA) of or higher. Satisfactorily complete 67% of credits attempted (pace of completion). Will graduate within 150% of the credit hours required to complete their Academic program. For the complete Standards of Academic Progress policy, please go to the Student Service Center to request a copy or go online at SAP Policy. A student not making satisfactory Academic Progress and has been denied financial aid has the right to Appeal in writing by following the instructions on this form.
2 The Financial Aid Office is only able to consider appeals based on unforeseen, documentable extenuating circumstances that prevented a student from meeting satisfactory Academic Progress . The decision of the Financial Aid Appeal Committee is final. _____ INSTRUCTIONS _____ 1. Complete the Standards of Academic Progress Appeal form. 2. attach your typed statement and documentation to the Appeal form. 3. Submit the completed Appeal form, statement and documentation to Lakeland Community College. The form and all documentation should be submitted together, rather than separately. Submitting separately may cause the review of your Appeal to be delayed or may even be denied. 4. Keep a copy of your Appeal and all documentation for your records. 5. Students who are continuing their education through the partnership program with CSU or Franklin will also need to complete a Bridge Course Form.

Appeals are reviewed in the order they are received, based on the date that all required documentation was submitted.
4 Appeals submitted during a term in which the student is enrolled will not be reviewed until grades are posted. If a decision has not been made on your Appeal by the time tuition and fees are due, it is the student s responsibility to pay the balance or make payment arrangements. If payment or payment arrangements have not been made, the student may be dropped from classes. Once a decision has been made on the Appeal , a message will be sent to the student s Lakeland email. The Appeal status may also be viewed through the student s myLakeland account. To check your SAP status on myLakeland: 1. Log into 2. Under the Student Tab click on Financial Aid 3. Click on My Eligibility select current aid year and submit 4. Click on Academic Progress _____ DEADLINE _____ Appeals are due no later than the first day of the semester.

6 2. Provide an Academic plan that includes a list of classes still needed to earn your degree/certificate at Lakeland Community College. Please use the Degree Tracker on your myLakeland account go to the Student tab and click on Degree Tracker. Under Format, select Student View and print the list of required courses needed to complete your program. 3. attach a copy of any documentation that will be helpful to the committee reviewing your Appeal . 4. If this is the only reason you are submitting an Appeal , continue to Section C. HOWEVER, if your cumulative GPA is below or your pace of completion is below 67%, you must continue to SECTION B. 5. Students who are continuing their education through the partnership program with CSU or Franklin will also need to complete a Bridge Course Form.

Students are strongly encouraged to speak with an Enrollment Support Specialist at the Student Service Center to make sure all applicable semesters are listed. Failure to include an explanation and documentation for all semesters where SAP requirements were not met may result in your Appeal being denied. 8 Section A Complete this section if you are requesting an extension of the 150% Maximum Timeframe policy. Section B Complete this section if your cumulative GPA is below and/or your pace of completion is below 67%, or you did not successfully complete any of the courses within the term (ALFAIL). Standards of Academic Progress Appeal Form 2. attach a typed statement. The statement must include an explanation of the following: What were the unforeseen, extenuating circumstances that occurred during each semester listed that prevented you from completing all of your classes? The statement must clearly show that the situation was beyond your control. What has changed that will allow you to successfully complete all classes in the future. 3. attach documentation that confirms the circumstance you indicated.
9 Provide documentation that verifies or supports the circumstances that are shared in your statement. All documentation must be official and from a third party professional. Examples of third party professionals include employers, doctors, judges, government officials, counselors, or clergy. Letters from third party professional must be on letterhead and/or notarized. If the extenuating circumstance is a situation that involves another person you must provide documentation of their relationship to you. The Appeal committee will only review the materials you provide; therefore, it is your responsibility to provide adequate documentation to support the circumstances as described within your Appeal . Appeals submitted without the typed statement or supporting documentation will not be reviewed.
10 The Information provided on this form, in my written statement and all accompanying documentation is complete and correct to the best of my knowledge. I agree to provide additional documentation if requested by Lakeland Community College. If my Appeal is approved, I understand that I must meet the following criteria each semester until I am in good standing according to the Standards of Academic Progress policy: 1. Successfully complete (grades of A, B, C, D, S, SA, SB, SC) in all the courses attempted during the semester. (Applies to ACPLAN and MAX PLN status) 2.
