Provide weekly, on-site support to the Participating Teacher by providing guidance, assistance, and information that leads the Participating Teacher to effective professional practice and movement through the FACT process.

3 Work with the Participating Teacher to develop, reflect upon, and revise a working Individual Induction Plan (IIP) document that is based on the California Standards for the Teaching Profession. Participate in Formative Assessment for California Teachers (FACT) training and other professional development activities for Support Providers. Assist the Participating Teacher in maintaining evidence of teacher and student growth by completing the FACT modules. Assist the Participating Teacher in the development, reflection upon, and revision of an Individual Induction Plan based on the California Standards for the Teaching Profession. Collaborate with other Support Providers, Content Specialists, experienced colleagues, Staff Development Specialists, and BTSA Director to assist with Beginning Teacher Support. Conduct informal and formal classroom observations. o BTSA classroom observations conducted by the Support Provider are confidential.

4 It is the Participating Teacher s decision to share information from observations. BTSA is not used for evaluation of personnel. Regularly visit and review the TUSD BTSA website and attend meetings that pertain to Support Providers. Participate in the program evaluation process by providing professional and constructive feedback when requested. Submit Support Provider Organizational Log to the Staff Development office twice a year. I have read the BTSA Support Provider responsibilities described above and discussed them with the BTSA Director. I understand I will receive half of my payment in January for Logs and IIPs turned in by December 10th and the balance in July for Logs and IIPs turned in at the spring Colloquium. Payment of the stipend is dependent on my submitting the Support Provider Organizational Logs and IIPs to the BTSA office twice a year. BTSA Support Provider: _____ _____ signature date _____ _____ _____ School home phone employee ID number 2 Tracy Unified School District BTSA Induction Program Support Provider Application APPLICATIONS DUE: NO later than 4:00PM, August 21, 2015 to Staff Development BTSA Induction Program Support Providers A Support Provider is a caring, supportive teacher willing to assist a Participating Teacher (PT) who is a participant in the BTSA Induction Program.

5 The intent of the program is to match a PT with a supportive member from the PT's grade level, content, or education specialist area. Pedagogical needs and local context are considered in the match. The SP position is a minimum two year commitment, the length of time a Participating Teacher spends in the BTSA Induction program. After four consecutive years of service, Support Providers will be asked to submit a current letter of recommendation from their Site Administrator. Support Provider Selection Criteria: SP s are assigned on an as-needed basis. The following criteria are considered when determining Support Provider and Participating Teacher assignments: Participating Teacher availability Similar grade level, content area, or education specialist authorization Same or proximal School sites Participating Teacher input Support Providers receive an honorarium of $1,500, which is distributed as follows: $ honorarium will be issued by the end of January $ honorarium will be issued upon completion of all Year 1 or Year 2 program requirements * The honorarium is subject to pro-rate and adjusted issuance date according to time and length of assignment ( Participating Teachers hired late or leaving the District during the year) as determined by the BTSA Induction Program Leadership Team.

6 Selection criteria are consistent with the support provider s specified roles and responsibilities, including but not limited to the following: (i) Knowledge of beginning teacher development; and knowledge of the state-adopted academic content standards and performance levels for students, state-adopted curriculum frameworks, and the California Standards for the Teaching Profession (CSTP); (ii) Willingness to engage in formative assessment processes, including non-evaluative, reflective conversations about formative assessment evidence with participating teachers; (iii) Willingness to share instructional ideas and materials with participating teachers; (iv) Willingness to work with participating teachers as demonstrated in Effective interpersonal and communication skills; (v) Demonstrated commitment to personal professional growth and learning; and (vi) Willingness and ability to be an excellent professional role model relative to ALL elements of CSTP 6: Developing as a Professional Educator.
