Our Healthy Incentives program supports you in living a healthier lifestyle by providing Incentives for you to:1. Register with Castlight (you must complete this step to be eligible for Credits )2. Know Your Numbers so you can stay on top of your health status3. Be Tobacco Free or make an effort to quitWhen you and your spouse/domestic partner enrolled in an Eaton medical plan take the required actions, Healthy Incentives Credits give you up to $75 per person per month toward medical plan are updating our programs to give you the benefits information you need when and where you need it: at home, at work and on the go.

2 Watch for new apps and programs starting in January!* NEW DEADLINES: NO EXTENSIONS. Because we will be changing some programs in 2018 , YOU MUST complete all actions by the following deadlines: November 13, 2017 to receive full-year Credits for 2018 December 31, 2017 to receive Credits starting March 1, 2018 For either deadline, if enrolled in the Tobacco Cessation Program, you must complete four program sessions by December 31, Register with Castlight You can only receive Healthy Incentives Credits when you are registered by the deadline. Castlight is an online resource that helps you manage your health care spending. If you haven t already, you and your spouse/domestic partner must register with Castlight to receive your Credits : Visit Castlight online through the left navigation on or by going to , Download Castlight Mobile for your smartphone, or Talk to a Castlight Guide by calling 1-855-572-2164 between 8:00 and 9:00 Eastern Know Your Numbers Earn your credit by registering with Castlight and completing Parts A AND B by the A: Know Your Numbers Complete a Know Your Numbers screening with Quest Diagnostics at your work location or at a Quest Diagnostics lab: Schedule online at , click on Eaton On-site Screening and Quest Diagnostics Lab Scheduler.

3 Print your confirmation page and bring it to your screening; or Schedule by calling you can t go to a Quest Diagnostics on-site or lab screening, you can use the test results completed by your doctor between January 1 and the your doctor a Physician Result Form available on through the left navigation. Your doctor must complete the entire form, and you or your doctor must fax or upload it to Quest Diagnostics. Keep a copy of the form and the fax/upload confirmation. All measurements and the 2017 screening date must be recorded on the form. A lab printout will not be accepted in place of a completed form. Screenings are preventive care, so if you are in an Eaton medical plan you shouldn t be charged for an in-network office visit unless other services are provided.

4 ANDPart B: Health Assessment Starting September 1, 2017, complete the Health Assessment questionnaire: Go to , then click Health Assessment on the Healthy Incentives Dashboard and answer all the questions (tambi n disponible en espa ol); or Call ActiveHealth at 1-877-489-9391 (para espa ol, presione 9) and give your responses to an ActiveHealth team YOUR NUMBERS POTENTIAL MONTHLY SAVINGS$25 credit for you.$25 credit for your covered spouse/domestic action to earn your Credits .. starting September 1, Be Tobacco Free Earn your credit by registering with Castlight and completing Part A OR Part B by the A: Certify you have been tobacco free for four months. Starting September 1, 2017, you can certify: Online at , click the Non-user Certification link on the Healthy Incentives Dashboard; or By calling an ActiveHealthteam member at 1-877-489-9391 (para espa ol, presione 9).

5 Even if you ve never used tobacco in your lifetime, you need to certify each year that you are tobacco free to receive the Healthy Incentives credit and qualify for lower Eaton employee optional life insurance rates (where available). ORPart B: Enroll in the Tobacco Cessation Program between January 1 and the deadline if you are not tobacco free. Complete four program sessions by December 31, 2017. Call ActiveHealth at 1-877-489-9391 to enroll. Remember to allow at least four to six weeks to complete four program sessions by December 31, 2017. Enrolling in the Tobacco Cessation Program does not qualify you for lower employee optional life insurance rates (where available). If you (and/or your spouse/domestic partner) aren t able to participate in the Tobacco Cessation Program and need an accommodation, call ActiveHealth to see if you qualify for an alternative TOBACCO FREE POTENTIAL MONTHLY SAVINGS$50 credit for you.

6 $50 credit for your covered spouse/domestic you (relatively) new to the Eaton medical plan?The rules for receiving Healthy Incentives Credits are different when you are a new employee. And specific rules apply to your spouse/domestic partner if he or she is new to the Eaton medical plan. You and your spouse/domestic partner automatically receive Healthy Incentives Credits through December 2018 if you were hired August 1, 2017 or spouse/domestic partner automatically receives Healthy Incentives Credits through December 2018 if you add your spouse/domestic partner to medical coverage: Due to marriage or another qualifying life event that occurred August 1, 2017 or later; or During annual enrollment for coverage effective January 1, you re not required to take action this year, the Healthy Incentives programs are still available to you.

7 For your own health and well-being, we encourage you and your spouse/domestic partner to register with Castlight, participate in the screening, take the Health Assessment and, if you use tobacco, enroll in the Tobacco Cessation Program.* ARE YOU RECEIVING AUTOMATIC Credits NOW? If you and/or your spouse/domestic partner are currently receiving automatic 2017 Healthy Incentives Credits , you both must take action by the deadline to earn Credits in your eyes on the DashboardThe Dashboard is opening on September 1, 2017, so get ready to start earning your Credits . As you complete your Healthy Incentives activities, you can watch your progress on the Healthy Incentives Dashboard on Remember, even if you know your numbers and have certified you are tobacco free, you will not earn your Healthy Incentives Credits until you register with Castlight.

8 A checkmark showing you ve completed your activities will show up on your Dashboard: Approximately three weeks after you register with Castlight (If you have already registered, you should see your checkmark when you first log in.) Immediately after you complete the Health Assessment and certify you are tobacco free online Within two weeks after: your Know Your Numbers screening you or your doctor s office faxes or uploads a Physician Result Form to Quest Diagnostics you complete the Tobacco Cessation ProgramSo check out your Dashboard, and if your Know Your Numbers and/or Tobacco Free activity doesn t show as completed, contact ActiveHealth at 1-877-489-9391; or if your Castlight registration is missing, contact Castlight at 1-855-572-2164.

9 Follow up as soon as appropriate to find out if you need to do anything else to earn your Credits . Don t miss the you don t have Eaton medical coverage ..We encourage you and your spouse/domestic partner to complete all of the Healthy Incentives activities this year, even if you aren t in an Eaton medical plan. It s good to learn about your health and potential risks. Plus, a mid-year change in your circumstances could mean you and/or your spouse/domestic partner will need Eaton medical coverage and you can t earn 2018 Credits after the deadline. And certifying your tobacco-free status is the only way to qualify for lower employee optional life insurance rates where they are available.
