ELIGIBILITY GUIDELINES Very low income VETERAN FAMILIES Honorable or Other than Honorable Discharge Homeless or at risk of Homelessness Currently living in their home & at risk of eviction OR currently homeless and scheduled to move into housing within 90 days OR have become homeless in the past 90 days SERVICES PROVIDED TO FRANKLIN, GADSDEN, JEFFERSON, LEON, LIBERTY, MADISON, TAYLOR AND WAKULLA COUNTIES IN FLORIDA AND THOMAS, DECATUR AND GRADY COUNTIES IN GEORGIA AND INCLUDE Case Management Development of a Housing Stability Plan Assistance to obtain benefits Referrals to community agencies Limited Financial Assistance Based on Need May include rent, utility, and deposits PROGRAM OFFICE LOCATED AT: 3333 W.
