Plan notes and Special Provisions may also determine additional and/or modified requirements or procedures. Mat er ial s Manuals may be purchased from the Department by contac ting the certification office. Upon purchasing the manual, the holder will be furni shed with a set of rev isi ons any time a rev isi on is published whi ch usually happens once a OF MATERIALS CERTIFICATION AND TESTING PROCEDURESR eviewing PlansBased on a review of the pr oj ec t plans, the Certification Engineer pr epares a DOT-14 whi ch is generated from the Materials Sampling and Testing system (MS&T).

3 The DOT-14 is a compi lat ion of the sampling, testing and certification requirements for each contract and the frequency at which they need to be Attachment 1 Building the DOT-14 Requi remen ts on the DOT- 14 come from the Minimum Sample and Test Requirements (MSTR) section of the Mater ial s Manual with the ex ception of mat erial s used in bi d items no t prev iously used by the Departmen t (somethi ng that is tot al ly new). In that case, the specifications ar e usually prov ided in the pl ans and/or special pr ovisi ons for that wor k. The DOT- 14 is accessible on the computer at al l region/ar ea offices.

4 Hard copy of the DOT- 14 can be pr int ed at any of these of the DOT-14 can be made avai lable to the contractor at the discretion of the Area/Proj ec t Engineer. The DOT-14 is a working document and is subject to change due to construction change orders, shop drawings and apparent omissions which may occur without notice. While a reasonably accurate review has been made of all dimensions and of compliance with contract plans and specifications, any corrections, comments or omissions of corrections and comments on the DOT-14 do not relieve the Contractor of their responsibility of complying with the requirements of said contract plans and specifications including changes which have received prior approval by the Engineer.

5 Certification and Testing RequirementsThe en tire pr ocess outlined herei n for certification and tes ting of material s is used in conjunction with the MS&T system. Nearly all of the forms used by fiel d personnel ar e automated within the system. Copies of other for ms not automated withi n MS&T can be obtained from the region/ar ea office . All fiel d tes t da ta will be loaded el ectronically. Sampl e data sheets will be pr epared within MS&T. The sy st em will assign a sampl e/ce rt ID nu mber for all mat er ial s and the data will be updated on the DOT-14 electronically.

6 When the wor k be gins on the pr ojec t, copies of al l certifica tion documents (certifica tes of compliance, certified mill test reports) received from the Contractor mus t be forwar ded to the Cer tification Engineer as they are received. Submittal of certifica tion documen ts will require the pr eparation of a cover let ter SDDOT CONSTRUCTION MANUALPROJECT MANAGEMENT SECTIONCHAPTER 3 - MATERIALSPAGE 3 - 3 REVISED JANUARY 2017indicating what documents ar e enclosed so that compl iance to specifica tion can be deter mine d. Letters for each material item selected by the Engineer can be printed from MS&T for submittal.

7 If all materials in the submittal meet applicable specifications, the Certification Engineer approves the documentation and inputs a date on the DOT-14 that indicates to the field personnel the item met specifications. If there is a deviation or the submittal is incomplete a letter is drafted to the Project Engineer, indicating what was missing or what is out of Letter Attachment 2 Yel low Dev iation Letter Attachment 3 The number of tes ts is updated each time a new test is entered and prepared. The number of certificates of compl iance is updated each time an approv ed supplier cert is added to the syst em or the certifica tion office approv es a ce rt pr ovided from a source not on the approved products list.

8 Tes ting forms for mater ials tested in the fiel d do not need to be for warded to the Cer tification Engineer. Sampl es of mater ial s which cannot be tes ted in the fiel d (liquid asphalt, ai r entrai ning agent, water reducing agen t, cement, cu ring co mpound, quality tes ts for aggregates) must be obtai ned and forwar ded to the Central Testing Laboratory for testing. Each sampl e must be accompanied by a DOT-1 (Sampl e Data Sheet) and may require a DOT-2 (Submittal Envelope). The DOT-1 should identify the mat erial , its int ended use, project nu mber, PCN, county, sa mple number, by whom submitted and to whom the results should be reported.

9 A copy of the certificate of co mpl iance received from the Contractor for the mat erial represented by the sampl e along with the cert ID number should accompany the DOT-1 whi ch is sent with the sampl e to the Central Tes ting Laborator y in Pierre. The DOT- 1' s and cert ID's are created in MS& Attachment 4 DOT-2 Envel ope Attachment 5 Certificate of Compliance. Attachment 6 Not e:A tabulat ion of the visual inspection requirements can be print ed out and used as a checklist to keep trac k of what documentation has or has not been of Visual Inspection Requirements. Attachment 7 Each material has a specific container in which samples should be placed.

10 The type of container can be found in the MSTR under the applicable material to be sampled.
