Earn $150 in wellness dollars when you complete the HEALTH Assessment and submit your results from a biometric screening to HEALTH ASSESSMENTThe online HEALTH Assessment is a questionnaire designed to give you an overview of your current HEALTH status and potential risks. To complete the HEALTH Assessment, log in to and select healthy Living from the top navigation, then HEALTH Assessment. DEADLINE: DECEMBER 31, 2018 BIOMETRIC SCREENINGA biometric screening can help you pinpoint HEALTH risks or identify areas to improve. The screening measures your height, weight, blood pressure, blood sugar, triglycerides and cholesterol.

3 To get the most accurate results, you ll need to fast before the screening. Complete your biometric screening at a participating lab or with your personal physician. DEADLINE: SEPTEMBER 30, 2018To select a biometric screening option and print the required form, if applicable, log in to and select healthy Living, then Screenings. If you see a personal physician for your biometric screening, select an in-network provider. Inform your doctor that the preventive office visit and lab testing related to the biometric screening should be coded as preventive services to ensure the screening is covered at 100% by the medical plan. If you get your screening at a participating lab, do not use your medical benefit the cost will be covered by the company. EARN $150 Take Action:Beyond gaining valuable insight into your HEALTH status, the wellness program encourages participants to take action and start building healthy habits. Choose and complete any four activities in this category and earn $75 each, or a maximum of $300.

4 Options include: IMPROVEMENT ON BIOMETRIC SCREENING RESULTS: If you completed a biometric screening in 2017, and you get a biometric screening by September 30, 2018, you will earn $75 for each biometric value that improves by 10%.MY HEALTH ASSISTANT GOALS: Use My HEALTH Assistant, an online tool offered through WebMD, to set and achieve healthy goals, such as increasing physical activity or improving your ACTIVITIES (APPROPRIATE FOR YOUR AGE AND GENDER): Log in to and select Rewards from the top navigation to see the preventive activities or screenings recommended for your age and gender. To earn wellness dollars for completing a preventive screening, check with your physician and Wellmark to determine if you re due for the screening and ensure it will be covered by your benefit plan. DEADLINE: DECEMBER 31, 2018To find a complete list of activities, log in to and visit the Rewards page. EARN UP TO $3003 TAKE CHARGE OF YOUR HealthBe Active: Stay healthy throughout the year by participating in quarterly wellness challenges through WebMD.

5 The challenges are designed to help you reach or maintain your HEALTH goals. Each challenge is held for four to eight weeks and focuses on increasing physical activity or preventing holiday weight gain. You may participate in all four challenges, and you ll earn $75 in wellness dollars for each of the first two challenges you complete, up to $150. 2018 Wellness Challenges include:READY! SET! MOVE! 8-WEEK ACTIVITY CHALLENGE, JANUARY 22-MARCH 18 Record an average of 120 minutes of physical activity each week or a total of 960 minutes throughout the challenge. If earned, wellness dollars will be distributed in April. DEADLINES: REGISTER BY FEBRUARY 2 AND ENTER ALL ACTIVITY BY MARCH UP 4-WEEK CHALLENGE, APRIL 23-MAY 20 Record an average of 5,000 steps per day or a total of 140,000 steps throughout the challenge. If earned, wellness dollars will be distributed in June. DEADLINES: REGISTER BY APRIL 27 AND ENTER ALL ACTIVITY BY MAY ! SET! MOVE! 4-WEEK ACTIVITY CHALLENGE, JULY 16-AUGUST 12 Record an average of 120 minutes of physical activity each week or a total of 480 minutes throughout the challenge.

6 If earned, wellness dollars will be distributed in September. DEADLINES: REGISTER BY JULY 20 AND ENTER ALL ACTIVITY BY AUGUST DON T GAIN 4-WEEK CHALLENGE, OCTOBER 29-NOVEMBER 25To help prevent holiday weight gain, enter your daily weight in your HEALTH Tracker and answer Yes to the challenge question for at least 21 out of 28 days during the challenge. If earned, wellness dollars will be distributed in December. DEADLINES: REGISTER BY NOVEMBER 2 AND ENTER ALL DATA BY NOVEMBER details will be provided prior to the challenge start dates. EARN UP TO $150If you think you may be unable to complete a wellness program activity that is required to earn wellness dollars, work with your personal physician to complete and submit the Wellness Program Waiver. To access the waiver, log in to and select Rewards, then All Rewards. If there is an error in wellness dollars, the company may make adjustments to correct the amount, which may include withholding amounts from your ALTERNATIVESThe wellness program is intended to help you achieve your best HEALTH . Rewards for participating in the wellness program are available to eligible employees and spouses. If you think you might be unable to meet a standard for a reward under this wellness program, you may qualify for an opportunity to earn the same reward by different means.
