Contract Clauses: All provisions and clauses from the respective NCO 8 NORTH MATOC IDIQ contract are applicable to this requirement, except for the following: a) 134 Display of Hotline Posters b) Market Research c) Liquidated Damages d) Notice and Assistance Regarding Patent and Copyright Infringement.
2 VHA Directive 2011-036 Safety and Health during Construction, effective September 22, 2011, applies to all construction activities performed at VHA owned or leased facilities regardless of whether performed by VHA or contractor staff. This solicitation references some of the contract clauses from the NCO 8 NORTH MATOC IDIQ. If there are any conflicts between the contract clauses and the information outlined in the resultant task ORDER , the MATOC IDIQ contract language takes precedence over the information in the task ORDER . 2. Service Disabled Veteran-owned Small Business Verification: Subsequent task orders are 100% set asides for verified Service Disabled Veteran Owned Small Business (SDVOSB). The award of this requirement shall not be delayed due to loss of SDVOSB verification. Proposals submitted by non-verified MATOC SDVOSB shall be excluded from award consideration.
3 Reference Section 10 of the SF1442 for the basic MATOC IDIQ contract. SDVOSB Verification The Government will review VetBiz ( ) to confirm Service Disabled Veteran-owned Small Business 9 SDVOSB) status verification for the submitted proposals. Evaluations will be performed on proposals submitted by verified SDVOSBs only. Subsequent confirmation of SDVOSB verification status will be performed for the apparent awardee. 3. Offer Acceptance Period: Offers providing less than 120 calendar days for Government acceptance after the date offers are due may not be considered and may be rejected. 4. Requests for Information: Requests for Information (RFIs) shall be emailed to until 2:00pm EST on May 24, 2012. Responses will be provided in a Question & Answer format to all NORTH MATOC holders. Do not send RFIs via the vendor portal-this is not an acceptable method and will not be responded to by the Contracting Officer.
4 5. PROPOSAL Due Date: Proposals to perform the work described herein are due by email to NLT 2:00pm EST/local time June 6, 2012. Reference: Sections and the basic MATOC IDIQ contract. 6. Offer Guarantee (Bid Bond): (An offer guarantee is required).Per FAR Clause Bid guarantee, paragraph C the amount of the bid guarantee shall be 20 percent of the bid price or $3,000,000, whichever is less. Note: a scanned copy of the original bid bond submitted with the PROPOSAL is acceptable, followed by the original in hard copy. Reference: Section of the basic MATOC IDIQ contract. 7. Site Visit: An organized site is scheduled for: Wednesday, May 16, 2012 at 10:00AM. Only one site visit will be conducted. Participants are to meet at Engineering Conference Room EB106-1, at the Malcom Randall VA Medical Center, 1601 SW Archer Road, Gainesville, Florida 32608.

Magnitude of Construction: Pursuant to FAR (d) the following is provided: The magnitude of construction is between $1 Million and $5 Million. 9. Work Element: This project is work element No. 3 and includes a 100 % design package. Reference: Section of the basic MATOC IDIQ contract. 10. Technical proposals are required in response to this REQUEST . II. PROPOSAL PREPARATION AND EVALUATION INFORMATION 1. PROPOSAL Preparation Instruction: To assure timely and equitable evaluation of proposals, offerors must follow the instructions contained herein. Proposals must be complete, self-sufficient, and respond directly to the requirement of this solicitation. Each offer in response to this REQUEST shall consist of the following: Sign offer (last page of these instructions) signed by the SDVOSB 51% owner and dated.
6 Price Offer Schedule - Statement of PROPOSAL Items must be filled out and a Price PROPOSAL , with detailed and itemized cost breakdown, must be attached and returned. PROPOSAL that addresses each evaluation factor. Basis for Contract Award Reference: Section of the basic MATOC IDIQ contract. SOURCE SELECTION USING THE LOWEST PRICE TECHNICALLY ACCEPTABLE PROCESS (LPTA). The Government intends to evaluate proposals and award a contract without discussions (except clarifications as described in FAR (a)). Therefore, the offeror s initial PROPOSAL should contain the offeror s best terms from a price and technical standpoint.
7 This is a competitive Task ORDER PROPOSAL REQUEST that represents the best value to the Government conducted under FAR Part 15, Lowest Price Technically Acceptable (LPTA) source selection process. Award will be made to the responsive, responsible, lowest priced, technically acceptable offeror that meets or exceeds the acceptability standards for all non-cost factors, based on availability of funds. SDVOSB Verification- The Government will review VetBiz ( ) to confirm Service Disabled Veteran-owned Small Business (SDVOSB) status verification for the submitted proposals. Evaluations will be performed on proposals submitted by verified SDVOSBs only. Subsequent confirmation of SDVOSB verification status will be performed for the apparent award. Evaluation Overview: Proposals will be evaluated and ranked according to price, from lowest to highest.
8 The Government reserves the right to evaluate only the technical PROPOSAL of the lowest priced offeror. Other technical proposals may not be evaluated unless the Government determines the lowest price offeror to be unacceptable or non-responsive. In the event the Government determines the lowest price offeror to be unacceptable, the technical PROPOSAL of the next lowest price offeror will be evaluated. This process will continue until a technical PROPOSAL is determined to be acceptable. 3. Offers will be evaluated utilizing the following Technical evaluation criteria: Technical Factor: The Technical factor consists of one element: Experience. Technical proposals will be rated either acceptable or unacceptable. This element must be rated acceptable to receive an overall acceptable technical rating. Proposals must separately address each element to be considered responsive to this solicitation with the exception of Past Performance, which requires no submission by offeror on this task ORDER PROPOSAL REQUEST .
9 Management Experience Offers shall address the following sub factors in narrative. (5 pages Maximum) 1. Provide documentation of prime contractor oversight of specific projects (at least 2) successfully completed or in progress that was coordinated and performed by the general contractor in the last 5 years similar in size ($1,000,000 to $5,000,000) and complexity. Contractor shall provide the following information for each contract/project submitted for evaluation: a. Company name/address/Phone # (Client) b. Project Title c. Brief Project Description d. Total Contract Amount e. Period of Performance f. Contracting Officer and COTR names and Phone # s and email address. 2. Provide General Contractor s Key personnel, duties and responsibilities in relation to this project.

Provide a list of subcontractors that the general contractor intends to use for this proposed project and include sub contractors relevant and recent experience (within the last 5 years) on projects of similar scope. 4. Past Performance- The Government will utilize the recent past performance evaluations used for award of the basic MATOC contract for this task ORDER /project in addition to the Past Performance Information Retrieval System (PPIRS), Dun and Bradstreet, Excluded Parties List System, Central Contractor Registration, Online Representations and Certifications Application, and other publically accessible databases. 5. Contractor Responsibility: NOTE: A determination of Contractor Responsibility will be accomplished for the apparent awardee prior to processing the award. This requirement is a pass/fail and is not a Technical Evaluation factor.
