These valuable appointments are wasted.

3 Please let us know if you cannot attend. Repeated failed appointments may result in you being removed from the doctor s list. GP. REGISTRAR We are proud to be an approved Training PRACTICE for Doctors who wish to become General Practitioners and usually one such Doctor is attached to the PRACTICE . These Doctors have Hospital experience but a requirement of the GP training is to spend a year in a Training PRACTICE before taking up a position as a General Practitioner. During this period some consultations may be recorded by video camera. This will only be done with the full understanding and written consent of the patients involved. From time to time we also host Student Doctors, District Nurses and Midwives. This is also an essential part of their training. They offer information and support to aid recovery and can be accessed via your GP RECEPTIONISTS Joanne Wood, our Reception Supervisor, along with the receptionists are a vital link in our communications.

7 All staff are committed to exceed the needs of our patients and sometimes may need to ask for additional details regarding your condition in order to help provide you with an appointment with the most appropriate health care professional to deal with your condition. We as a PRACTICE support the NHS Zero Tolerance policy in Primary Care regarding aggressive/abusive behaviour. ADMINISTRATION AND SECRETARIAL TEAM Cath Houten is the Deputy PRACTICE Manager assisting the manager in her duties. The admin team comprises of a Medical Secretary, Data Manager, Data Manager s assistant, Administrator and Notes Summariser. These team s duties include: the call and recall of all patients attending Chronic Disease Management Clinics ensure that all information received from the hospital for patients who have either been in-patients or out-patients is recorded and actioned by the appropriate members of the PRACTICE Ensure all patients notes are kept up to date and accurate Ensuring all referral letters are prioritised and sent to the appropriate departments.

8 ATTACHED STAFF health VISITORS The health Visitors attached to this PRACTICE can be contacted on 951086 to give you confidential advice on all aspects of health for families and young children. DISTRICT NURSES The District Nursing Service aim to provide high quality effective nursing care in the community to meet the needs of patients and their carers. Their main aims are to: > provide a visiting service to deliver care to patients with identified nursing needs for those who are housebound. > Offer specialist-nursing care for acute and terminally ill patients > Work with the PRACTICE Team to maintain the health of patients by promoting their independence. The District Nurses can be contacted direct on 657526. COMMUNITY MATRON/NURSE Sue Rowe is the Community Matron and Gillian Crossley is the Community Nurse that works with the PRACTICE to monitor patients in their own homes who suffer from chronic disease conditions.

COMMUNITY MIDWIVES The team of Midwives visit and care for the pregnant and post natal women in their homes and give their advice and support at the Ante Natal clinics in the surgery. The Community Midwives are based at fleetwood Hospital and can be contacted by calling 306068. SCHOOL NURSES Pauline Ronson Alison Hamer Lucy Fenucciu The School Nurses are involved with our patients and work closely with the Primary health Care Team. Each School has a named nurse. The School Nurse is trained to identify the needs of the school age child and maintains an important link between the school, parents and community care. The School Nurses are based at fleetwood Hospital and can be contacted by calling 306066. SMOKING CESSATION CLINIC Individual appointments are available at the surgery and treated with utmost confidence.

10 Discussion includes information about all the products available on NHS prescription. A free copy of a book to help you deal with the psychological addiction or habit is available. The specialist advisors are very positive and helpful and can give you plenty of time. Self Referral can be arranged by booking an appointment with the receptionist or by telephoning 01524 845 145. HOW TO ACCESS A MEMBER OF THE PRIMARY health CARE TEAM We run a computerised appointment system. Although you register with one of the doctors by name, you can be seen by any one of the partners, providing there is an appointment available. It may, on occasion, be more appropriate to see one of the nurses or the pharmacist depending on your symptoms.
