1 The Polar Express Charity Train Non-Profit Ticket Application Form Must be submitted by September 30, 2016 Email completed applications to Organization Name: Primary Contact Name: Address : City: Phone : State : ZIP: Fax : E-mail : Mission: People Served : Number of Polar Express Tickets Requested (Maximum of 50): Please explain why your organization desires tickets aboard the Polar Express Charity Train in 500 words or less. Attach additional pages as necessary. Charities awarded must be end-users of tickets provided. Charity Train tickets may not be used for raffles, auctions, giveaways or similar fundraising or promotional events.
2 The Polar Express committee plans to review all applications and award tickets by October 7, 2016. All applicants will be notified once selections have been made.
