2 Wears proper footwear, ( OSHA approved footwear) Hazardous Communication: Reports & documents hazardous conditions at jobsite Has access to Material Safety Data Sheets according to company policy Only uses approved TKE chemicals and all are properly labeled, (including secondary containers) Fire Protection & Prevention: Has properly dated & sized fire extinguisher (A,B,C type), ( 2lb. for all vehicles, 5lb. for gang boxes, 10lb. whenever hot work is being performed) Hand & Power Tools: Understands to tag defective tools & equipment with Out of Service or Do Not Use tags Uses hand and power tools according to manufacturers recommendations, TKE policy & OSHA standards Welding & Cutting Uses flashback arrestors & check valves when using oxygen/acetylene Separates & secures oxygen/acetylene cylinders as required (OSHA approved storage when not in use) Stairways, Ladders & Barricades Properly uses ladders according to manufacturers recommendations Proper barricades are in place and being used Uses pit ladder or portable ladder when entering & exiting pit Scaffolding: Uses scaffolding according to manufacturers recommendations & OSHA standards NO SCAFFOLDING UNLESS APPROVED IN WRITING BY VP OF OPERATIONS Vehicle & Trailer Safety.

3 Operates company vehicle or trailers in safe condition, ( no broken windshield or mirrors, uses seat belt, operative brakes, horn, turn signals, etc.) Material Handling: Uses safe lifting practices when lifting materials Other Areas: Uses ThyssenKrupp Installation, Service & Maintenance processes Uses Project Management Book Has adequate lighting in and around hoistway & corridors Uses Risk Assessment form when deviating from standard processes All electrical covers & mechanical guards are in place Hoistway, machine room, pit, gang boxes, company vehicles, etc. are kept clean & organized TOTAL POINTS: 99+1 = PERFECT SCORE POSSIBLE POINTS: 100 Note: Any rating of 0 or 1 in this Safety Assessment Form requires the issuance of a Safety Citation to the responsible field employee by the Supervisor/Manager performing the assessment.
