2 If your application orrecertification forms contain false or incomplete information, you may be:CEvicted from your apartment or house;CRequired to repay all overpaid rental assistance you received;CFined up to $10,000;CImprisoned for up to 5 years, and/or;CProhibited from receiving future State and local governments may have other laws and penalties as QuestionsWhen you sit down with the person who fills out your application, you shouldknow what is expected of you. If you do not understand something, say so. That person can answer you question or find out what the answer theApplicationIncomeWhen you give your answers to application questions, you must include thefollowing information:CAll sources of money you and any member of your family receive(wages, welfare payments, alimony, social security, pension, etc.)
