There are penalties that apply if youknowingly omit information or give false information. Penalties forCommitting FraudThe United State DEPARTMENT of HOUSING and URBAN development (HUD)places a high priority on preventing fraud.

2 If your application orrecertification forms contain false or incomplete information, you may be:CEvicted from your apartment or house;CRequired to repay all overpaid rental assistance you received;CFined up to $10,000;CImprisoned for up to 5 years, and/or;CProhibited from receiving future State and local governments may have other laws and penalties as QuestionsWhen you sit down with the person who fills out your application, you shouldknow what is expected of you. If you do not understand something, say so. That person can answer you question or find out what the answer theApplicationIncomeWhen you give your answers to application questions, you must include thefollowing information:CAll sources of money you and any member of your family receive(wages, welfare payments, alimony, social security, pension, etc.)

3 ;CAny money you receive on behalf of your children (child support,social security for children etc.);CIncome from assets (interest from a saving account, credit union, orcertificate of deposit; dividends from stocks, etc.);CEarnings from second job or part time job;CAny anticipated income (such as a bonus or pay raise you expect toreceive).HUD-1140-OIGTHIS DOCUMENT MAY BE REPRODUCED050307 AssetsCAll bank accounts, savings bonds, certificates of deposit, stocks, realestate, etc., that are owned by you and any adult member of yourfamily/household who will be living with business or asset you sold in the last 2 years for less than its fullvalue, such as your home to your names of all of the people (adults and children) who will actuallybe living with you, whether or not they are related to theApplicationCDo not sign any form unless you have read it, understand it, and aresure everything is complete and you sign application and certification forms, you are claimingthat they are complete to the best of your knowledge and belief.

4 Youare committing fraud if you sign a form knowing that it contains falseor misleading you give on your application will be verified by yourhousing agency. In addition, HUD may do computer matches of theincome you report with various Federal, State or private agencies toverify that it is must provide updated information at least once a year. Some Programsrequire that you report any changes in income or family/householdcomposition immediately. Be sure to ask when you must recertify. 5 Reporting AbuseIf you are aware of anyone who has falsified an application, or if anyone triesto persuade you to make false statements, report them to the manager of yourproject or PHA.
