The term PCC will be usedthroughout this Directory . Generally, you must getyour health care services from your PCC. You cansee any Primary Care Physician (PCP) at your network providers listed in this Directory haveagreed to provide you with your health care may go to any of our network providers listed inthis Directory . You can get all covered services onyour own, without a referral. If you have been goingto one network Provider , you are not required tocontinue going to that same , even network doctors are not aware thatthey should send you to a network hospital forinpatient care.

7 If you need to go to a hospital, workclosely with your doctor to choose the mostappropriate facility for your needs from the hospitalsin the Ucare Medicare Plans some cases, you may get covered services fromout-of-network providers. The Point-of-Service(POS) benefit provides access to non-contracted,out-of-network Medicare providers. You do not needa referral from your PCC to use the POS benefit. Fordetails about POS refer to your Evidence of Coverage(EOC) for the exception of emergencies, or urgent care,or out-of-service-area renal dialysis, or some otherservices, it may cost more to get care fromout-of-network must use network providers except inemergency or urgent care situations or forout-of-area renal dialysis or other services. If youobtain routine care from out-of-network providers,neither Medicare nor Ucare Medicare Plans will beresponsible for the you receive needed care from a Provider whois not part of the plan network, you are onlyresponsible for paying your share of the cost, not forthe entire you pay the entire amount yourself at the timeyou receive care, you need to ask us to pay youback for our share of the cost.

8 Send us the bill,along with documentation of any payments youhave times you may get a bill from a Provider askingfor payment that you think you do not owe.
