IT WILL BE THE CONDITIONS OF ANY ENTRY AT THE UPHA CHAPTER V TO HOLD BLAMELESS ANY PARTIES CONNECTED TO THE SHOW FOR ANY TYPE OF LOSS. EVERY CLASS OFFERED HEREIN, WHICH IS COVERED BY THE RULES AND SPECIFICATIONS OF THE CURRENT USEF RULEBOOK, WILL BE CONDUCTED AND JUDGED IN ACCORDANCE THEREWITH. ENTRY DEADLINE: All entries close April 24, 2017.

3 Post Entries will be accepted based on stabling availability. STABLING IS LIMITED and post entries will only be accepted if stabling is available. ONE OWNER PER ENTRY FORM: Entries must be completed on the entry forms furnished by the UPHA CHAPTER V HORSE Show. PLEASE TYPE OR PRINT. HORSES must BE ENTERED BY THEIR REGISTERED NAME AND CORRECT OWNERSHIP. Copies of registration papers are required. YOUR SOCIAL SECURITY OR FEDERAL TAX ID NUMBER IS REQUIRED TO RECEIVE A PREMIUM CHECK OPEN ACCOUNTS: There will be no open charge accounts without an open check or credit card on file in the office. SIGNATURE: Every exhibitor, owner, agent or trainer must sign the entry blank and be accompanied by a check or credit card covering all fees and charges.

4 ALL ENTRIES are required to have a Certificate of Equine Infectious Anemia Test (Coggins Test) performed within 12 months of show. Health papers are required on all out of state horses. VACCINATION REQUIREMENTS 1. Horses entering the grounds must be accompanied by documentation of Equine Influenza Virus and Equine Herpes Virus (Rhinopneumonitis) vaccinations within six months prior to entering the stables. Horses not in compliance with this rule may be required to leave the competition grounds upon request by Competition Management. SHIP IN DATE: Sunday Afternoon, May 7, 2017 STALLS: HORSE and tack stalls are $135 each for the entire show. Late Entry Stalls $155. NO FIRST BEDDING PROVIDED. All stalls are 10 X 10 temporary box stalls and equipped with swinging doors.

5 STABLING IS LIMITED and post entries will only be accepted if stabling is available. BEDDING: NO OUTSIDE BEDDING ALLOWED. All bedding must be ordered through HORSE Show Office. See entry form. A fee will be charged for shavings removal if bedding is brought in by an exhibitor. CAMPERS: Camper Fee $15 per day first come first served. TIME OUTS: Only one (1) 5-minute timeout may be requested per exhibitor per class. In case of casting a shoe or breakage of equipment in the paddock, the gate may be held no more than 3 minutes. There will be no delay after the first HORSE enters the ring, at which time the gate will be held for 2 minutes. THE HORSE SHOW COMMITTEE decision shall be final and shall decide all questions not covered in these rules and regulations.

6 CONTROL OF HORSES: Horses must be kept under control at all times. At the discretion of the judges and/or steward, horses may not be allowed to compete. Owner will forfeit entry fees. SHARPS CONTAINERS: Competition management may fine any individuals including trainers, owners, exhibitors, or their agents up to $100 for improper disposal of needles or other sharp disposable instruments. See RING DIMENSIONS: Show Ring 107 feet x 217 feet sand /loam footing Covered Show Arena. Covered Make up Area 90 feet by 180 feet Sand/loam Surface. COURTESY TO OFFICIALS: Any act of discourtesy to the judges on the part of any exhibitor, owner, trainer or groom shall disqualify the HORSE and rider. The owner shall forfeit his entry fees and any prize money won at this show.

7 During the time of judging, no person will be allowed in the ring except ring attendants, judge(s) of the class and when necessary the handler in charge of the HORSE being judged. Attendants or grooms will enter the ring in suitable attire only. Headers may enter the ring by the end gate only and must take no actions that will affect the performance of any animal in the class. CANCELLATION/SCRATCHES: Exhibitors canceling their entries after the official closing for any reason shall forfeit the accumulation of entry and stall fees. All scratching of horses at the show must be done at the show office one hour before the start of the session in which they are to compete. PLEASE MAKE SCRATCHES EARLY AND BE CONSIDERATE TO OTHER EXHIBITORS. CHAMPIONSHIPS: To be eligible horses must have been entered, shown and judged in any other class of its division or section.

8 The HORSE Show officials may exercise their rights to limit Championship/Stake Class entries to the ribbon winners from the qualifying classes. VETERINARIAN: The Official Veterinarian will be on call and will inspect such horses designated by the judge(s) or steward to be considered unsound and will pass on any question pertaining to the soundness of any HORSE before being excused from the class. Their decision will be final. They will be available at all times and telephone numbers will be posted on the door of the HORSE Show office for after show hours. PAYMENT OF PREMIUMS: All prize money will be paid by check drawn to the order of the owner as listed on the entry blank. Checks will be forwarded by mail within 30 days after the show. SOCIAL SECURITY NUMBER OF THE OWNER must BE INCLUDED ON ENTRY FORM BEFORE PAYMENTS ARE MADE.
