2 Certain personal information is required to be on the SYSTEM to allo w yo u to complete your Enrolment . The SYSTEM is, however, very secure and exceeds industry security standards. Do I have to make my benefit selections the first time I log onto the flex Enrolment web site? No. You can easily try out different combinations of benefit selections before making a final decision. You can make selections, close the SYSTEM and return within the Enrolment period to make further changes. The SYSTEM will show your last saved selections; you may t hen continue the process after considering your needs before you submit your final selections. No further changes can be made once you have finalized your selections. You will be asked to Submit your Enrolment .

3 Once you "submit", your Enrolment is finalized and no further changes can be made. If you do not submit , your benefits will not be considered to be submitted and will therefore be subject to the default provisions of the plan . If I make a mistake when I enroll, can I go back and make a correction? The Enrolment period is open for a specified period. You can try as many benefit combinations as you like during this period; however, once you have submitted and finalized your selections by Submitting Enrolment again your choices are locked in until the next allocation year. Will I be able to log back on and check my selections after the Enrolment period closes? Yes, you can view your benefit allocations any time. My Enrolment information is incorrect.

4 Whom do I contact? Contact your Benefits Representative. How do I know my Enrolment is complete? Once you make your selections, you will be asked to confirm and submit your choices. Please follow instructions carefully to ensure that your submission is completed properly. You will receive an email confirmation immediately once your selection is finalized. If you do not receive a confirmation email, your submission has not been completed and you must return to the SYSTEM to complete it. Please ensure you are certain about your choices before submitting as you cannot make changes on the SYSTEM once your Enrolment has been submitted and confirmed. I completed my Enrolment ; what happens next? If you elected Optional Life or Critical Illness coverage that requires evidence of insurability for yourself, yo ur spouse or your children, you must print the necessary Medical Evidence of Insurability forms from the SYSTEM .

5 Complete and sign the forms and mail them to the address indicated on the forms. If you changed your Health and/or Dental coverage, a new Alberta Blue Cross ID card will be mailed to you. If you allocated credits to a Health Spending Account and/or a Personal Spending Account, your credits willbe deposited into the respective account(s) with Alberta Blue you allocated credits to a Registered Retirement Savings plan (RRSP) or a Tax Free Savings Account (TFSA) for the first time, you should immediately access the Standard Life link on the flex Enrolment web site to complete your application; determine your investment selections; and designate a beneficiary. It is necessary to complete an Enrolment with Manulife in order to have your funds deposited into aregistered account.
