2 Our benefits enrollment system has the record of your current plan, and if you are in the HMO plan, we are obtaining your current Primary Care Physician (PCP) information from Blue Shield, both of which will be transmitted to Aetna for you and your dependents, so your enrollment is completed, and your new ID card(s) will be issued in June, for the July 1 effective date. Note: This change will not affect the Kaiser HMO to Open Enrollment! May 1 May 31, 2018 Most major doctor groups are fully in Aetna s network: Alamitos, AppleCare, Brookshire, Cedars Sinai, Good Samaritan, Harriman Jones, HealthCare Partners, Hoag, Lakeside, Lakewood, Long Beach Memorial, Memorial Care, Orange Coast Memorial, PrimeCare, Prospect, Regal, Riverside, St. Joseph, St. Mary, Torrance, UC Irvine, and UCLA, among many of CareOn the off chance that you are currently receiving care or treatment for a condition described below from a doctor or provider who is not part of the Aetna network, a waiver may be available for the following: Pregnancy, if confirmed prior to July 1, 2018 Cardiac rehabilitation Physical therapy, occupational therapy or speech therapy Radiation treatment or chemotherapy Post-surgical care for surgery that was performed prior to July 1, 2018 Terminal Illness where life expectancy is six months or less Carryover hospitalizationsTo find out if you are eligible for transitional care, you must call Aetna at (888) 290-6620 prior to July !

3 NEW ID Cards If on an Aetna plan, a new ID card will be mailed to your home address. Please show your new card to providers for any services you receive on or after July 1, 2018. If you don t show your new card, your claims could be Spending Account Enrollment and Plan YearWe re moving FSAs to a midyear benefits cycle this year starting July 2018. Our new FSA plan year will be July 1 June 30. Enroll for FSAs during this Open Enrollment period. If you don t enroll now, your next opportunity will be in May 2019 for the July 1, 2019 June 30, 2020 plan year.
