2 (b) Chapter 13 Debtor s Duties. In addition to the dutiesimposed on a chapter 13 debtor by the Bankruptcy Code, the FederalRules of Bankruptcy Procedure, applicable non-bankruptcy law andelsewhere in the Bankruptcy Local Rules and general Orders, thefollowing duties are imposed on chapter 13 debtors:(1) Documents Required by chapter 13 Trustee. Not laterthan fourteen (14) days after the filing of the petition orconversion of the case, Debtor shall provide to the chapter 13 Trustee:(A) Form NDC 1-2 (Domestic Support ObligationChecklist), or written notice of the name and address ofeach person to whom debtor owes a domestic support 1 UNITED STATES BANKRUPTCY COURT 2345678910111213141516171819202122232425 26obligation, together with the name and address of therelevant state child support enforcement agency (See, 464 and 466).

3 (B) Form NDC 1-3 (Class 1 Checklist) for each Class 1claim; and (C) Form NDC 1-4 (Authorization to Release Informationto Trustee Regarding Secured Claims Being Paid by theTrustee).(2) Periodic Reports.(A) Upon the chapter 13 Trustee s request, debtorshall provide the Trustee with a copy of any tax return, W-2 and 1099 form filed or received while the case ispending, and furnish the Trustee with periodic financialinformation regarding the debtor s business or financialaffairs.(B) Debtor shall file with the bankruptcy court adeclaration signed under penalty of perjury stating debtorhas made the post-petition payments debtor proposed to bemade directly to each applicable named Class 1 Creditor,and attach to each declaration proper documentary evidenceof the payments made (the Declaration ).

4 Prior toconfirmation of the plan, such Declaration(s) shall befiled five days before the original and all continuedmeetings of creditors and any contested confirmationhearings, or continued contested confirmation hearings. The bankruptcy court will not confirm a Chapter 13 plan21 UNITED STATES BANKRUPTCY COURT 2345678910111213141516171819202122232425 26(and may dismiss the Chapter 13 case) if the Debtor is notcurrent on these post-petition/pre-confirmation payments. Post confirmation of each year the plan is pending, suchDeclarations shall be filed on a quarterly basis no laterthan January 20, April 20, July 20 and October 20.

5 Thefinal Declaration, however, shall be filed within twenty-one days of the last plan payment being received by theTrustee. The bankruptcy court may dismiss the Chapter 13case if the Debtor is not current on these post-confirmation payments.(C) Additional reports may be required by the IS SO : November 6, 2017_____Roger L. 7 A nonstandard provision will be givenno effect unless this section indicates one is included in section 7 and it appears in section alterations to form plan permitted. Other than to insert text into designated spaces, to expand tables toinclude additional items, or to change the plan title to indicate the date of the plan or that it is a modified plan, thepreprinted text of this form shall not be altered. No such alteration will be given any of collateral and lien avoidance. Unless otherwise provided in Section 7 below, as to non-governmental units, the confirmation of this plan will not limit the amount of a secured claim based on a valuationof the collateral for the claim, nor will it avoid a security interest or lien.

8 This relief requires a separate claimobjection, valuation motion or adversary proceeding, or lien avoidance motion, with supporting evidence, that issuccessfully prosecuted in connection with the confirmation of this plan. Determining the amount of secured andpriority claims of governmental units, however, must be done in compliance with 3012. of Plan. In the absence of a timely written objection, the plan may be confirmed without a hearing. It will be effective upon its 2. Plan Payments and Plan plan payments.
