ATTENTION APPLICANTSPLEASE READ THIS PAGE BEFORE COMPLETING THE ATTACHED APPLICATION FORCERTIFICATE OF ELIGIBILITY AND CONTRACTOR QUALIFICATION NOTE: Asset Management certification is only required for prime bids on projects for theconstruction, reconstruction, installation, demolition, maintenance or repair of any building by a publicagency estimated to cost more than twenty-five thousand dollars ($25,000). Asset Managementcertification is not required for filed FOR CONSIDERATION :1. Your firm must be in business for at least one (1) Your firm must have successfully completed a minimum of five (5) contracts in any one of thecategories of work listed on the Application for Certificate of Eligibility (Form CQ1) within thepast five years.
