Vehicle Driver (form F3712) issued in accordance with the commercial standards stated in the Assessing Fitness to Drive - Commercial and Private Vehicle Drivers medical standards. The certificate must be no more than six months old. Certificates not using form F3712 may not be accepted. Note: All criminal history must be declared regardless of time
