CORPORATE AMENDMENTS APPLICATION . Existing insurers use the Uniform Certificate of Authority CORPORATE Amendment APPLICATION for requesting AMENDMENTS to its Certificate of Authority. A Uniform State is one that is committed to using the Uniform Certificate of Authority (UCAA) review process for company licensing and admissions. The Applicant Company can use the CORPORATE Amendment APPLICATION to file more than one change in the same submission. The Applicant Company should mark all changes on the APPLICATION form and submit all items required for those changes. For electronic APPLICATION submissions, refer to the UCAA.
