After the post-grant innovation has been clearly defined, a plan for sustaining the necessary implementation supports, infrastructure, and funding can be developed. The following chart provides a hypothetical Example of a Sustainability plan for an evidence-informed innovation. It focuses on implementation and organizational infrastructure and processes, as well as the fiscal strategies and resources for Sustainability . Based on a plausible but fictitious scenario, the innovation targets anger management issues in youth who have been recently placed in out-of-home care.
