While guidelines are useful aids to assist providers in determining appropriate practices for many patients with specific clinical problems or prevention issues, guidelines are not meant to replace the clinical judgment of the individual provider or establish a standard of care.
Jul 23, 2019 · Use clinical judgment to assess whether level of hyperglycemia warrants adding insulin and metformin at the same time. If patient declines insulin, add metformin if not already taking. • If level of hyperglycemia warrants it, add glyburide at same time as metformin. • If patient is intolerant to metformin, switch to glyburide. Below 126 mg/dL
