Pharmacist LICENSE Application INSTRUCTIONS Renewal . This Application must be completed by pharmacists licensed in Maryland who want to renew their license in accordance with Md. Code Ann., Health Occ. 12-308. Complete the attached Maryland Board of Pharmacy's Application for Pharmacist Licensure Renewal . Submit the completed Application with all attachments and a check or money order made payable to the Maryland Board of Pharmacy (Board) in the amount of $ to: Maryland Board of Pharmacy, Box 1991, Baltimore, Maryland 21203-1991. Applications sent overnight or through priority mail must be sent to: Wells Fargo Bank, Attn: State of Maryland Board of Pharmacy, Lockbox 1991. 7175 Columbia Drive, Columbia, MD 21046. A total of 30 Continuing Education Credit Hours (CEs), obtained within the last two years, are required to be submitted at the time you apply for Renewal . Two (2) CEs must be live, one (1) CE. must be on medication errors. A CE is considered live if it offers the ability for the participant to have real-time interaction with the presenter.

LATE SUBMISSION REMINDER: A renewal application received at the Board that is postmarked after your current license expires will require you to reinstate your license.
