Whilst we have clearly set the Direction , there is still a long way to go before we have a health service in Wales that truly delivers the right care to the right person at the right time in the right place by the right challenge now is for all of us to embrace the opportunity that the current NHS reforms offer to deliver the paradigm shift towards community-based care, where safe and appropriate to do so.
