Supporting documentation requested
We need you to send us information to support it em s you claimed on your tax return. We are holding the Earned Income Credit (EIC), and/ or the Additional Child Tax Credit (ACTC) p o r t io n (s) o f yo u r refund pending the results of the audit. If you claim ed t he Pr e m i u m Tax Cr e d i t (PTC), we may also hold all or a part of your refund due to a discrepancy w it h your PTC. Be sure t o respond w it hin 30 days from the date of this notice or we ll disallow the items being audited, and you m ay o w e ad d it io n al t ax. What you need to do im m ediat ely Review t he list of it em s w e re audit ing and provide copies of documentation to verify what you claimed on your tax return. See the enclosed forms for complete instructions for what you need to send. Complete the Response form at the end of this notice, and mail or fax it to us along with any documentation within 30 days from the date of this notice.
