All rights reserved ISSN 1554-7752 THE Moura MINE disaster : A CASE OF FAILURE IN ORGANISATIONAL LEARNING AND KNOWLEDGE MANAGEMENT Judith Ann Chapman University of Western Sydney PENRITH SOUTH DC, NSW, AUSTRALIA Abstract The Moura case provides the facilitator with a rich source of material on the failure of managerial decision-making, due largely to poorly integrated mental models and knowledge management strategies. It also allows for the examination of related psychological factors on the capacity of individuals to assess situations and respond wisely. In particular, the impact of stress and anxiety arising from the hazardous operating environment is also broached, encouraging students to explore an emerging issue in managerial decision making.
