1 To Waive the Requirement to Purchase the Student health insurance plan 1. Have the following information available: a. 2 B. For your first time completing the waiver i. Select My Policies. Click on NEW. ii. Add the insurance policy information and click Submit. iii. Click on the I Have health insurance link or the I Have health insurance button. iv. Select the term and year for which you will be registering from the drop down menu. v. Click Submit. If * displays, you left out required information. If displays you have made an error in that entry. vi. If the green success indicator flashes, close this window. vii. Check your compliance by selecting My Compliance Records. If it does not show compliance for the terms you need, make sure you selected the correct term and year for the waiver. c. To renew your waiver for compliance: i. If your health insurance has NOT changed and there is no termination date shown, go to the I Have health insurance button.

3 Select the term and year for your waiver from the drop down menu. Click Submit. You should see the green success indicator. ii. If your health insurance has changed, go to My Policies. 1. Update your existing policy: a. Click on the pencil icon to the right of your policy information. b. Update the information and click Submit. c. Then go to the I Have health insurance button to complete the waiver. 2. Add a new policy: a. Click NEW. b. Add the new policy information. Click Submit. c. Then go to the I Have health insurance button to complete the waiver. iii. Check your compliance at My Compliance Records to make sure you are ready for registration. Student health insurance How to Purchase this plan : o Go to Log in at the flashing red and black link at the top of the page.

4 Select the Purchase FSU health insurance button. Follow the prompts. Understand that we do not accept that you purchased the plan by accident. There are too many opportunities to leave the process before you add the charges to your account. If you want to see premium costs, go to the home page of the insurance site and scroll down to the premium matrix at the bottom of the page. o Dependents may be covered under this plan . Add dependent information during the selection process. If you do not, their coverage will not be effective even though you have paid their premium. Dependent coverage must mirror your coverage period. Charges will be placed on your account at Student Financial Services. o If you have selected the annual plan , the first half of the annual premium will be billed on your fall charges.

5 The second half of the annual premium will be billed on your spring charges. o Payment is due when financial aid is disbursed. o If you do not want financial aid to pay for your health insurance , the premium must be paid at least 2 weeks before financial aid is disbursed. Payment may be made on line with a credit card. A convenience fee is charged. Credit card payments cannot be made in person or over the telephone. Payment may be made by mail via check or money order. Make the payment payable to Florida State University and mail the payment to UCA1500, Tallahassee, Fl 32306-2394. Include your name and emplID on the payment and indicate that the payment is for health insurance premium. Payment may be made in person at UCA1500 (the cashier s office). They accept cash, check and money orders.

6 O If you do not have sufficient financial aid to pay the entire insurance premium on the due date, no late fee is associated with the premium due. The charges remaining on your account must, however, be paid in full before your next registration window opens. IF YOU ARE PLANNING TO PARTICIPATE IN INTERCOLLEGIATE ATHLETICS, CONTACT THE COORDINATOR OF insurance /RISK MANAGEMENT AT THE FSU ATHLETIC DEPARTMENT AT (850) 645-2700. The school sponsored insurance will take care of your health care needs outside of participation in intercollegiate athletics. The school sponsored insurance will not cover your participation in intercollegiate athletics. How to get a copy of your insurance card: United HealthCare Student Resources (UHCSR) will send you an e-mail notification with instructions on how set up your MyAccount with them.

7 At the site, you can o Print a paper copy of your insurance card, o Get the card via an app for a smart phone or o Request a hard copy of your card via the mail. Make sure your e-mail address is up to date in Student Central and is marked as your preferred e-mail. If it is not your notification will not get to you. You can update your mailing address in Tallahassee on your MyAccount but you must also update the address in Student Central to make the change permanent. If you do not, the weekly upload will change your address at the carrier back to the one in Student Central. How to find a provider off campus: Go to your MyAccount at Select Locate Network Provider. You will see two options. o UHC Choice Plus is the provider network for medical care. o United Behavioral health is the provider network for mental health .

8 How to use your insurance card: Whenever you receive medical services, make sure to provide your insurance card to the provider. Make certain the provider knows to file your claim with Student Resources, not the standard United HealthCare. The address for claims in on the back of your insurance card. When you use the pharmacy you will pay o $20 or the cost of the medication, whichever is less, for generic medications, o $50 for brand name medications. o If you have questions about covered medications, the prescription drug list is on your MyAccount at View Prescription plan . View Value Added Benefits at your MyAccount is worth exploring. You can access the discount dental and vision options and so much more. It is worth your time to see what other benefits are part of your student health plan .
