All contractors will be selected based on their expertise of service and on their strength in environmental, health and safety performance and the ability to comply with safety and environmental laws and regulations in alignment with the PFIZER mission.

2 PFIZER will not consider contractors with poor safety and/or environmental performance and reserves the right to exclude such contractors and/or dismiss them, if necessary, from work at PFIZER Puerto Rico Region. PURPOSE This guideline is developed to establish a solid framework for the management and oversight of Contractor activities in regards to environmental, health and safety matters. It covers contractors, subcontractors, consultants and service representatives that perform on-site services for the Puerto Rico Region. At a minimum, contractors, subcontractors, consultants and service representatives will follow all applicable federal, state, and local laws and regulations, plus PFIZER applicable rules and guidelines.

3 Additional requirements may be requested based on the particular job to be performed at each site. SCOPE This guideline focuses on environmental, health and safety (EHS) requirements for contractors, subcontractors, consultants and service representatives. It is the intent of this guideline to be in alignment with PFIZER Global Manufacturing (PGM) Environmental, Health and Safety policies and with applicable PFIZER requirements. APPLICABILITY This guideline applies to all PFIZER Puerto Rico Region work activities involving contractors, subcontractors, consultants and service representatives ( Contractor ) who are performing on site services including but not limited to maintenance or repair, turnaround, renovation, new installations, construction, demolition, operational services or specialty work.

4 RESPONSIBILITIES On site service providers / contractors must comply with the following responsibilities: Applicable regulations according to the Puerto Rico Occupational Safety Office and 29 CFR 1926 and 29 CFR 1910 standards. EFFECTIVE date : 12/5/2007 PFIZER REVISION: PAGE: 2 of 6 PFIZER TITLE: Contractor PRE-QUALIFICATION ENVIRONMENTAL HEALTH AND SAFETY (EHS) GUIDELINES Applicable regulations according to Environmental Quality Board, Environmental Protection Agency and Department of Environmental and Natural Resources. Provide a written environmental, health and safety program prior to commencing work that includes the following elements: management Commitment Employee Involvement Work location analysis Hazard Prevention and Control Safety and Health training including certificates of trainings.

5 Environmental Pollution avoidance /control This program must meet all applicable government requirements. Have a company substance abuse policy and documented program. Contractor employees must comply with PFIZER s substance abuse policy. Provide a copy of the Workmen s Compensation Insurance Policy (Puerto Rico - Fondo del Seguro del Estado ) for the specific job to be performed and prior to commence that particular job. Contractor shall submit a Safety Plan at the beginning of a significant task. The plan must describe how the work will be accomplished and the safety precautions to be followed, and must be appropriate to the scope and complexity of the work involved.

6 The safety plan will normally be a part of the overall job plan, but it may also be a separate document. Personnel must be properly trained to provide adequate safety and craft skills for the work. (Competent persons). Contractors must submit evidence of those trainings. All heavy equipment operators must be properly trained and certified, and must use equipment that is safe to operate and which meet all federal, state and local requirements. All Contractor employees must attend the PFIZER EHS orientation prior to commencing work or as required. Provide personal protective equipment including but not limited to fall protection systems, lanyards, hard hats, safety glasses, protective clothing, gloves, etc.

7 Provide a minimum of 30hrs OSHA Outreach Program training for construction industry and general industry to all safety personnel and/or foremen. EFFECTIVE date : 12/5/2007 PFIZER REVISION: PAGE: 3 of 6 PFIZER TITLE: Contractor PRE-QUALIFICATION ENVIRONMENTAL HEALTH AND SAFETY (EHS) GUIDELINES Contractor Safety Representative shall manage the Contractor s safety program and have the authority to correct any hazard or unsafe practice on the project, even if a work stoppage is required. This representative must report directly to the Contractor s corporate safety manager and/or designee. A full time safety representative is required for jobs having 10 employees or more at the site.

8 Ensure that all Contractor employees and subcontractors understand the hazards and safety requirements for their work activities. Contractors must keep records of their safety performance including their subcontractors in accordance with OSHA requirements. (Incident investigations, JSA, OSHA 300, list of employees who have received safety trainings, etc.) Safety management of the Contractor s employees is the responsibility of the Contractor . Provide Tool Box meetings on a weekly basis. Meeting and attendance must be properly documented. Obtain any safety approvals required by PFIZER (hot work, lock out / tag out, confined space entry, line breaking, excavation, roof access and general permits, etc.)

9 Prior to commencing work. For fire alarm or sprinkler system work, notify assigned PFIZER representative 24 hours in advance of the time the system is scheduled to be interrupted. Subcontractors must be pre-qualified by PFIZER prior to perform on-site work. Provide MSDS of materials that will be delivered to the site (including compressed gases). Activities involving materials such as glue, paint, tar, etc. must be coordinated with PFIZER prior to commencing work. Consider ventilation and respiratory protection if applicable. Must have a certified firewatcher for each hot work area with appropriate training on extinguisher use. Provide a safety monitor for fall protection if needed. All scaffolds must be erected, modified or dismantled by trained/certified scaffold builders and must be inspected daily by a competent person.

10 Certification tags shall be affixed to all scaffolds. Portable toilets are required for contractors, if applicable. Provide facilities for drinking water, hand cleaning, and flushing of the eyes and body for immediate emergency use when accidental exposure to corrosive materials occur. All electrical equipment must be connected to GFCI provided by the Contractor . For jobs that represent a risk to employees, a Job Safety Analysis (JSA) must be submitted prior to commencing work.
