2 Prescription drug co-payments are $10 / $30 / $50 / $150 for specialty drugs. The vision hardware allowance for the Core plan is $75. The coinsurance maximum has been eliminated and the total out of pocket maximum will increase to $7,350 in accordance with the Affordable Care Act. Plus The Plus plan will offer a $850 ANNUAL deductible which is increased from FY 17-18 and a 15% coinsurance. The primary care office visit co-payment will increase to $20 while the specialist co-payment will remain $35. The emergency room co-payment is $200 and the Urgent Care co-payment is $40. Prescription drug co-payments are $10 / $25 / $40 / $150 for specialty drugs. The vision hardware allowance for the Plus plan is $125. The coinsurance maximum has been eliminated and the total out of pocket maximum is $6,600. High Tech Diagnostic Services (examples include but are not limited to: MRI, CAT / PET Scans, MRA, Echo, Nuclear Medicine) will continue to be applied to deductible and coinsurance without regard to place of service for the Core and Plus plans.

3 Express Scripts will continue to serve as our pharmacy benefit manager (PBM). Effective 7/1/ 2018 , Express Scripts will be implementing the Walgreen s Smart 90 Program. This program allows members to receive a 90 day supply of maintenance medications at a times copay when purchasing at a Walgreen s Pharmacy. Those opting to purchase their 90 day maintenance medications at other retail pharmacies will be limited to a 30 day supply at the regular monthly copay amount. Additionally, Johnston County will be participating in Express Scripts Advanced Utilization Management program. This program will introduce enhanced step therapy, prior authorization and drug quantity limits for new prescriptions written on or after 7/1/ 2018 . Aetna Behavioral Health will continue to serve as the Mental Health and Substance Abuse benefit coordinator. Johnston County will continue to provide Teladoc as a free benefit at no cost to you.

4 Teladoc is a convenient alternative to urgent care or ER visits. board-certified physicians licensed in North Carolina are available anytime, anywhere and can resolve many non-emergency medical issues via phone, skype, facetime or webcam. When medically necessary, these physicians can prescribe medications for a wide range of conditions. You and your household dependents have access to Teladoc regardless of your Medical plan ENROLLMENT . 5 VVOOLLUUNNTTAARRYY DDEENNTTAALL PPLLAANNSS MetLife will continue as the dental insurance carrier. MetLife will offer both High plan and Low plan UCR options where dental services are based upon payments made in accordance with usual, customary and reasonable charges (UCR). Both plans offer the opportunity to visit any dental provider of choice; however, you could save money by seeing a participating dentist. A brief overview of the major benefits of each plan follows: The High plan offers a $50 ANNUAL deductible with a $1,250 ANNUAL benefit maximum. Preventive services will be paid at 100% of UCR, Basic services will be paid at 80% of UCR after deductible and Major services will be paid at 50% of UCR after deductible. Orthodontia will be paid for dependent children only at 50% of UCR with a $1,000 lifetime benefit maximum. The Low plan offers a $50 ANNUAL deductible with a $750 ANNUAL benefit maximum.

6 Preventive services will be paid at 90% of UCR, Basic services will be paid at 60% of UCR after deductible and Major services will be paid at 30% of UCR after deductible. Orthodontia will be paid for dependent children only at 30% of UCR with a $1,000 lifetime benefit maximum. Employees/dependents not currently covered that wish to join the plan July 1st will be considered late entrants and will only have coverage for exams, cleaning and child fluoride for the first 12 months. Expenses may include office visit co-payments, deductibles, coinsurance, prescription co-payments, chiropractic care, vision expenses, and dental/orthodontia care. The Dependent Care FSA allows you to pay for qualified childcare centers, preschools, and before and after-school care on a pre-tax basis. Additionally, please be aware that even if you participated in the FSA over the past year, you must re-enroll for the new plan year that begins on July 1, 2018 . Healthcare FSA maximum ANNUAL contribution amount increasing to $2,650. Dependent Care FSA maximum ANNUAL contribution amount remains $5,000.

8 VVoolluunnttaarryy SShhoorrtt TTeerrmm DDiissaabbiilliittyy JJuullyy 11sstt,, 22001188 OOppeenn EEnnrroollllmmeenntt Provided by Unum As an active Johnston County employee working 30 hours or more per week, you are eligible to purchase voluntary short term disability insurance. The voluntary short term disability insurance provided by Unum covers up to 60% of your weekly salary with a maximum of $600 per week. The benefit, should you become disabled as a result of a non-work related illness or injury, will begin following a 30-day elimination period and will continue up to 52 weeks. Should you have elected the Voluntary Short Term Disability plan during your new hire ENROLLMENT process, then you can now receive up to a maximum of $600 in coverage per week, salary permitting. If you did not previously enroll in this benefit, now is your opportunity! This is a one-time Open ENROLLMENT offered by Unum. This allows you the opportunity to purchase Voluntary Short Term Disability coverage without having to answer any health questions (guarantee issue).

9 If you do not elect coverage at this year s Open ENROLLMENT and wish to apply at a later date, you will be required to answer health questions and Unum can either accept or decline coverage.
