2 D) Soldier contact information collected and provided to DOL and VA representatives to support successful transition including employment e) DOL and VA Representative information provided to Soldiers with instructions to contact these personnel to pursue employment transition support and attain CRS. 3) Signature of DD Form 2958. a) Soldier (Block 28a). b) ACAP Counselor or State Transition Assistance Advisor (TAA) (Block 29a). c) Commander or designee (Block 30a). 4) Administration a) DD Forms 2958 consolidation and upload into iPERMS. b) Final storage of documents WHO: All Soldiers who have served more than 180 days on T10. ACAP Counselors and TAAs update ACAP XXI records for Soldiers State assigned VOW POC(s) coordinate training, track completion, and support admin duties WHEN: capstone will occur following completion of all required components of TGPS (for exempt Soldiers this can occur at DEMOB).

3 Completion for Non-exempt Soldiers must occur following the Department of Labor Employment Workshop (DOLEW) prior to REFRAD. If CRS is not met, follow up at the first Yellow Ribbon event to assist with meeting CRS. WHERE: capstone can be held at DEMOB, home station, or Yellow Ribbon. WHY: capstone is a quality assurance check to ensure Soldiers meet program requirements and receive the support needed from local resources to successfully transition to civilian employment.
