6 BMC TRANSPORT APPLICATION 2018 -19 (All sections MUST be completed) PAYMENT INFORMATION THE TOTAL COST OF YOUR BMC TRAVEL PASS WILL BE 475 FOR 2018 -19. Please tick one of the payment options:- 4 instalments by Direct Debit (3 @ 120, 1 @ 115) Please complete the attached Direct Debit Mandate. Three instalments of will be taken on or around 1st October 2018 , 1st December 2018 and 1st February 2019. The final instalment of will be taken on or around 1st April 2019. In full by credit/debit card Our Finance Team will contact you for your payment details before the beginning of term.

7 Please note that your bus pass cannot be issued until payment is received. IMPORTANT INFORMATION COLLECTION OF BUS PASSES You will be notified regarding final pick up points and timetables during August 2018 . Centrebus Passes will be posted to the address on this APPLICATION . Passes for college contract routes will be part of the student ID card and will need to be collected at Enrolment. You should therefore ensure that you are available to attend on your enrolment day. EVEN IF YOU ARE NOT REQUIRED TO ATTEND FOR ENROLMENT YOU MUST ARRANGE TO COLLECT YOUR STUDENT ID CARD/BUS PASS BEFORE THE BEGINNING OF TERM.

8 You will not be able to travel on your first day of induction without your bus pass. DEADLINE FOR ALL applications 31st July 2018 Do not wait for your exam results, or your seat can not be guaranteed! If you change your mind, and you no longer need your bus pass due to withdrawal from the course, you MUST contact TRANSPORT Services to cancel your pass (before the beginning of the academic year) to ensure that Direct Debits are cancelled. BURSARY SCHEME You may be entitled to bursary assistance with your travel costs if you qualify. Conditions apply.

STUDENT/PARENT DECLARATION *I (insert name of student) unconditionally accept that BMC is not responsible for providing alternative TRANSPORT where unavoidable delays/route changes are necessary due to unforeseen circumstances, road traffic accidents, traffic jams or adverse weather. BMC reserves the right to vary routes and/or pick-up points/times for organisational or safety reasons.

10 Bus passes remain the property of BMC and may be removed in the event of any breach of learner or college agreement, attempted or suspected fraudulent use of the pass, the pass being damaged or interfered with and/or non-payment of all or part of the college TRANSPORT fee. I understand if I miss my TRANSPORT for any reason and arrange TRANSPORT of my own, I will not be reimbursed by the College. I understand that the travel pass should be carried at all times.
