1 CHENEY TRAFFIC SAFETY EDUCATION PROGRAM PROGRAM requirements * PROGRAM INCLUDES DOL WRITTEN TEST AND driving TEST AVAILABLE IN B&B CAR 1. The cost of the PROGRAM is $ for basic PROGRAM . $515 with skid monster (go to ). May be paid by cash, check or credit card. This may be made in payments consisting of $250 at time of enrollment and balance before the last week of class. $10 a month late fee will be charged for unpaid balances. Payment is made to B&B driving School and not the high school. Payment received date will determine registration for classes that fill.

2 No refunds. 2. Classes will be held on Monday / Wednesday. Class will be held at CHENEY High School in room 223. Student may not miss more than three scheduled classes or will be dropped from class. No refund will be given. PROGRAM must run at least 5 weeks and not exceed 15 weeks after the last day of class. Class schedules are listed on the back of this form. 3. Students must complete all learning activities as assigned by the instructor. Students will need a drivers guide in order to complete assignments. Donate for Life presentation will be presented on the first day of class.

3 4. Student must be 15 years old by the first day of class and obtain a driving instruction permit prior to the second week of class. A permit may be given as early as 10 days prior to class starting if your student is 15 years old, registered, and has a DOL ID#. Students must successfully complete behind the wheel driving activities. Xtreme Control Advanced driving Techniques Skid Control Training Just in time for B&B driving School Winter driving 466-2343 The Skid Monster (recently seen on KHQ News) The Skid Monster is a vehicle control simulator, a speed control simulator, and a traction loss simulator.

7 It s an exciting, challenging an unforgettable teacher. Drivers learn: * How vision keeps car control * How as little as 1 -2 mph of speed change can be the cause of a major skid. * To feel reduced grip between the road and tires, and where the envelope of traction exists. * Crash avoidance skills and reacting to secondary skids. * How to prevent, detect, and recover from adverse conditions. New drivers, experienced drivers, young drivers, and seasoned drivers, the Skid Monster gives you the opportunity to practice correct behavioral techniques. The Skid Monster allows drivers to gain confidence while experiencing failure and success in a safe environment.

8 This XTREME CONTROL encounter is for the entire family. requirements 1. Participant must have a valid Instruction Permit or Drivers License. 2. Complete the Release, Waiver, and Indemnification form. (on back of parent letter) 3. One hour training, three students per hour. 4. Training times will be available on designated weekends and school holidays. Call B&B office for times and dates of training. 5. Training is located at Spokane Fairgrounds. Take the south entrance on Havana Street, drive straight back until you see the gate opening with the Skid Monster sign on the right.

9 6. Re-scheduling for skid monster training must occur at least 72 hours prior to appointment. No refunds. RELEASE, WAIVER, AND INDEMNIFICATION FOR SKID MONSTER The undersigned participant and his/her parent or legal guardian (if under the age of 18 years) does hereby execute this release, waiver and indemnification for himself/herself and his/her heirs, successors, representatives and assigns; and hereby agrees and represents as follows: To release the B&B driving School, Inc., its members, employees, agents, representatives, agencies, other organizations and businesses affiliated with this course from any and all liability, loss damage, costs, claims, and/or causes of action, including but not limited to all bodily injuries and property damage arising out of participation in the Skid Monster Clinic, it being specifically understood that said PROGRAM includes the operation and use by the undersigned participant and others of automobiles.

10 The undersigned further agrees to indemnify the B&B driving School, Inc., its employees, members, agents, representatives, agencies, other organizations and businesses affiliated with this course, and hold them harmless for any liability, loss damage, cost, claim, judgment or settlement which may be brought or entered against them as a result of the undersigned s participation in said PROGRAM . This indemnification shall include attorney s fees incurred in defending against any claim or judgment and incurred in negotiating any settlement. It is understood and agreed that the undersigned shall have the opportunity to consent to any such settlement, provided, however, that such consent shall not be unreasonably withheld.
