1 SK-247 (4/27/2010) CLARIFICATION RE SSA 3RD PARTY VERIFICATION attempts Date: _____ Property: _____ Name: _____ Unit #: _____ If the Social Security benefit information in EIV agrees with the amount that the resident reports, management may use the EIV information as third PARTY VERIFICATION for calculating the resident's income and if applicable, the Medicare premium paid by the resident. We have attempted to obtain alternative 3rd PARTY VERIFICATION because: The resident disputes the EIV data The resident is unable to provide acceptable and current income documentation (HHS Data only) There is incomplete EIV data for the resident; or There is no EIV data for the resident If the information in EIV does not agree with the amount that was reported by the resident, or when the resident disputes the EIV data, management must ask the resident to provide a current Award Letter from SSA.
2 Information from the current Award Letter will be used as VERIFICATION . The Social Security Administration will not respond directly to the management office. The Social Security Administration did agree to provide updated benefit information directly to the resident. Resident was asked to provide documentation in compliance with HUD Handbook Revision 1 and instruction provided in the EIV Webcast Questions and Answers.
