1 Procurement Engineering Standard Purchase Clause Code: Title: QA PROGRAM CSA OR CSA R013 Applicable to: Items and services with CSA or CSA quality PROGRAM requirements This clause applies to purchase orders placed on or after March 19, 2018 Page 1 of 2 QA PROGRAM CSA or CSA requirements The supplier shall implement, maintain, and provide item(s) / service(s) in accordance with a quality PROGRAM as assessed and accepted by Ontario Power Generation as meeting the requirements of CSA or CSA and requirements specified in the Ontario Power Generation purchase order. The company shall extend all appropriate requirements specified in this purchase order to sub-tier suppliers. If the company determines any QA PROGRAM requirements specified in this order are not applicable to sub-tier suppliers, documented justification for this determination shall be available for review by Ontario Power Generation.

2 Inspection & Test Plan: The manufacturer shall prepare the Inspection and Test Plan (I&TP) document as part of the QA PROGRAM requirements. A copy of the I&TP, signed by the authorized QA representative as a minimum, shall be submitted to OPG with shipment. However, OPG review and acceptance of the I&TP prior to proceeding with fabrication is only required when specified by the Inspection and Test Plan Submission Requirement clause QA-I&TP specified elsewhere in the Contract Purchase Order. Note: The requirement of a completed ITP shall be the blank copy of the original ITP, signed and authorized by the QA representative (also applies to the referenced completed I&TP s in other Procurement Engineering Clauses - EQPR s, etc). The completed (working) copy of the ITP containing the manufacturing and inspection information shall be made available by the supplier for the purpose of OPG audits or source inspections A Certificate of Conformance is required.

3 Failure to provide one with complete and accurate information will result in your shipment being put into Quality Hold. A. Certificate of Conformance (C of C) applicable to Manufacturers: The company shall provide a similar statement on the packing slip or on a separate cover document with the following certification and information: 1. Conformance statement: "The equipment/item/material listed herein has been inspected by the company and is in conformance with the contract requirements and approved for shipment". 2. OPG Purchase order number, 3. OPG Catalogue ID number, 4. Printed name, title and handwritten signature or electronic signature that ensures traceability ( , Digitally signed ) of an authorised representative of the company s quality assurance authority. 4 When the Company is a Distributor, or a Manufacturer acting as a Distributor for product supplied from stock, there are two options: A manufacturer s Certificate of Conformance exactly per requirements in A noted above and a copy of the manufacturer s quality PROGRAM certificate. The manufacturer and exact facility location must be on the Distributor s supplier list on file with OPG. or A Certificate of Conformance from the Distributor exactly per the requirements noted in A above, with the following additions: identify on the C of C who manufactured the product, along with the manufacturer s exact facility location and the QA PROGRAM implemented by the manufacturer. Note: the purchase order may have a separate requirement for a Certificate of Compliance (usually pressure boundary material), which is different from the Certificate of Conformance.
