This Assessment will help you understand what steps you can take to improve or maintain your Health by creating a personal healthy lifestyle plan just for you. It also provides valuable information that you can share with your personal physician. your privacy is very important to us. Succeed is completely confidential and adheres to all HIPAA regulations.

Please note that the screen shots used in these directions may change without notice as the website is updated. How to Get Started: 1. Start by going to your Member Web site at 2. 3 CLICK HERE Page 2 Rev. 08/2009 3. Enter your USER ID and PASSWORD then proceed to step 6. If you do not have a USER ID and PASSWORD, create one by choosing REGISTER NOW from the LOG IN box and follow the instructions as directed. a. Please Note: If you have previously visited the Highmark Web site, but have forgotten your User ID and Password, you cannot create a different User ID and Password. Please contact the Login Help Line at 1-877-298-3918. This process will only take a few minutes. 4. Complete the REGISTRATION FORM.

4 For security reasons, your password must be at least six characters and use a combination of both letters and numbers. Select the SUBMIT button after confirming your password. CLICK HERE CLICK HERE Page 3 Rev. 08/2009 5. After you click SUBMIT, the following message will appear regarding your PERSONAL IDENTIFICATION NUMBER (PIN). Read and then click OK to continue. ENTERING your PIN NUMBER IN THE FUTURE After you receive your PIN via mail in 3 to 5 days, you should enter it in the space provided. By entering your PIN, you will have access to your confidential Health care information; however, the PIN is not required to access Health and wellness programs, including the Succeed Questionnaire .

5 6. After you establish your login, choose WELLNESS PROFILE under Quick Resources on the Welcome page. CLICK HERE Page 4 Rev. 08/2009 7. Choose START Questionnaire to take the Succeed Questionnaire . 8. Follow the Questionnaire to complete your wellness profile. If it takes more than 30 minutes to complete, the Highmark site will show a popup window asking if you need more time. Click YES, then minimize the Highmark window to return to the Questionnaire . 9. If you are unable to complete the Questionnaire in one sitting, you may log out and continue another time, as long as you DO NOT click on the SUBMIT button.

6 Log back onto your Member Web site and follow the previously outlined steps to return to the My HealthMedia page. You may continue the Questionnaire by clicking on CONTINUE Questionnaire . CLICK HERE Page 5 Rev. 08/2009 10. Complete the Questionnaire . At the end of the Questionnaire under the section, ADDITIONAL QUESTIONS, please leave the SITE CODE field blank. 11. You may review your responses in the Questionnaire before submitting it. Click on the section to review. When you are satisfied with your responses, click SUBMIT Questionnaire .

7 Once you submit your Questionnaire , you will not be able to redo it. 12. This screen will appear while your Questionnaire is being processed. IF YOU WERE GIVEN A SITE CODE, ENTER IT HERE.
