1 CV Application Form Summary NHS Jobs has introduced a new CV Application ' form, and some additional features that can be used in conjunction with the new form, or with the existing Application forms. The additional features include: the ability to make a CV attachment mandatory for a specific vacancy;. the ability to require an additional document to be uploaded;. the ability to include an additional question that contains a longer free text answer option ( up to 400 words). The CV Application ' form is available in the training environment if you would like to explore the feature further in a safe environment before using it on a live vacancy.
2 Choosing the CV Application form On the Add a New Vacancy' page when selecting which Application form should be used, there will be an option for CV Application . Allow CV attachment' will be selected automatically when this Application form is chosen (see below). This provides applicants with the option to upload their CV, if they wish. If you would like to make the CV attachment mandatory, select the relevant tick-box indicated in the screenshot displayed below. March 2017 1 NHS Jobs 2017. CV Application Form Sections The CV Application form contains the following sections for the applicant to complete: Personal Information Supporting Information Monitoring Information Safeguarding Submit and Declarations With the exception of Supporting Information', these sections will collect the same information as the existing standard and medical and dental Application forms.
3 In its default state, the Supporting Information' section of a CV Application ' form will simply provide the option for the applicant to attach a CV: March 2017 2 NHS Jobs 2017. If any non-safeguarding Application form additional questions are included for the vacancy, these will be added to the supporting information' tab of the Application form (safeguarding additional questions will be added to the safeguarding' tab of the Application form). The arrows on the screenshot below show how the supporting information page may look if additional Application form questions are added.
4 The examples below show additional questions with file upload' and extra-long free text' answer types. Also indicated is how the CV upload field will look if it is made mandatory. Application Form Additional Questions Two additional answer types have been made available for use with Application form additional questions: Extra-long free text (400 words). File upload (.doc, .docx, .jpeg, .jpg, .pdf, .png, .rtf, .xls, .xlsx). March 2017 3 NHS Jobs 2017. If you require another document in addition to the CV (such as an expression of interest document for example), this can be requested using an additional Application form question with a File upload' answer type.
5 The document types that can be uploaded are shown in the image above. Documents with a file size of up to 1MB can be uploaded. If any Application form additional questions are included on a vacancy, they become mandatory questions on the Application form. Additional questions using the new answer types can be used on vacancies using any Application form type they are not restricted to use with the CV Application ' form. Application form additional questions are created by the organisation's system administrators. Please contact your local system administrator(s) if you would like any additional questions created.
6 Offline (Word) Application Forms Word versions of the CV Application ' form are available to support offline applications. These are accessed by selecting Users' toolkit > Application Form from the Quick links'. section of the navigation menu.
