Minimum DWELLING coverage amount Broad form $25,000 Basic form $10,000 2. STRUCTURE A. Foundation Broad form: Continuous solid foundation Basic form: Block pier or wood post acceptable B. Heat Basic form: Space heat acceptable Broad form: Floor furnace or circulating heat acceptable up to $60,000 Central heat (thermostatically controlled, vented in every room) required for value in excess of $60,000 C.

A completed wood stove questionnaire & photo of stove must accompany application 5. UNACCEPTABLE RISKS OR EXPOSURES A. Vacant or unoccupied dwellings B. Wood stoves or kerosene heat as sole source of heat C. Vicious dogs, including any dog that has bitten previously or any risk with the following: Rottweiler, Pit Bull, Doberman Pinscher, German Shepherd, Chow, Akita or any dog mixed with these. D. Existence of trampoline, broken steps, missing handrails or unfenced pools E.

10 Applicants who have been canceled, non-renewed or declined by another company in the last 5 years F. Applicants who have had a total loss or theft claim G. Check with underwriting if applicant has had any losses H.
