1 Eagle Scout rank Application Checklist Review your Council website for specific instructions. If instructions in this Checklist conflict with your Council, use the Council instructions. 2 The dates in this report are your official BSA dates. Use them, not the dates from troop records. Using the form provided by your Council, choose five people who will recommend you for the rank of Eagle Scout (six, if employed). One must come from your parents or guardians, one from someone who can speak to your spiritual values, and one from your educational world. When you provide each of your references with a form, also provide them with a stamped envelope addressed to the Committee Chair of your unit. Your name should be on the outside of the envelope. For merit badges 7, 8 and 10 on the form, cross out the merit badges NOT earned. (For example, if you earned Swimming, cross out Cycling and Hiking. If you ALSO earned Cycling or Hiking, you can enter those merit badges as electives.) Enter leadership ONLY performed as a Life Scout . Make sure that your leadership start date is either on, or after your Life Scout Board of Review date.

3 Enter the total number of hours from your Eagle Scout Service Project Workbook. Write an essay as described in on page 2. This is, .. a statement of your ambitions and life purpose, and a listing of positions held in your religious institution, school, camp, community, or other organizations, during which you demonstrated leadership skills. Include honors and awards received during this service. A single page will do. Your board of review is interested in how your Scouting career affected your other activities and service. Have your committee chair sign and date your Application . Eagle Scout Service Project Workbook: All contact information is complete (page 1-6). Eagle Scout Service Project Workbook: Make sure that you have four signatures with dates, plus your signature and date, on the Project Proposal.

4 Eagle Scout Service Project Workbook: Plans, drawings, sketches, notes etc. should be attached behind your project plan. Eagle Scout Service Project Workbook: Proof of fundraising approval, as required by your Council. Eagle Scout Service Project Workbook: Hours entered in Service Project Data (Page 4-3), along with an attachment showing how that data was tracked (some sort of project log.) Eagle Scout Service Project Workbook: Insert one or two photos of the finished project into the Project Report where indicated. Eagle Scout Service Project Workbook: A signature and date from both your Unit Leader (Scoutmaster) and Beneficiary, plus your own signature and date. Eagle Scout Service Project Workbook: Exhibits should include sign-in sheets or other evidence of participations of others, receipts, and copies of a few thank you letters. A single page of photographs in chronological order is nice evidence of your project for your Eagle Board of Review.

5 Assemble everything as requested by your Council. They may request an original Application plus a copy, so be sure to read your Council instructions closely. Deliver to your Council office as per their instructions. This must be done PRIOR to your 18th birthday.
