2 Faculty: Dr. Paul Keckley New Post-Acute Initiatives Affirmant Health Partners is a Physician driven ACO, formed in 2015 and based in Southwest Michigan. They are the largest SCIN in Michigan consisting of 6 Health Systems, 5000 physicians and more than 25 Acute Care Hospitals. LeadingAge Michigan has been collaborating with Affrimant in a number of key areas critical the membership focusing on Post-Acute care and shared savings. In this session members will learn about ongoing, statewide, Post-Care Initiatives already in development as well the implementation of Clinically Integrated Networks (CIL s) being discussed by Affirmant Health Partners. Faculty: Karin Proos, Affirmant Health Partners Strategic Leadership in the Changing Environment In today s aging services market, strategic leadership for all elder care services must be a focus.

3 Organizations must remain in compliance with regulatory requirements, medical service environment and data analytics at CMS while managing their revenue with the changing payment systems, claims oversight and outcomes tracking. In an environment where data means everything to everyone come learn how to best prepare your organization for the scrutiny that is coming. Faculty: Leah Klusch, The Alliance Training Center FEATURED PRESENTATIONS ADRs, Audits & Documentation With the Centers for Medicare & Medicaid Services (CMS) increased focus on fraud, abuse and waste, it seems that there is an onslaught of audit contractors with varying focuses. This can be overwhelming for providers in our already heavily regulated profession. Knowing who is looking at the claims and how the audits are completed will shed light on the whys and will help providers better prepare themselves for the process.

4 Faculty: Maureen Hedrick & Jennifer Leatherbarrow, Richter Healthcare Consultants Become Unmistakable Starting the journey from commodity to oddity requires right-brained thinking, or operating from the heart. A lot of business rely on left-brained leadership, watching numbers, cutting costs, managing day-to-day operations. This session will challenge leaders to consider right-brained leadership which will lead to an employee-centric culture. Happy, engaged and empowered employees will naturally make for happy residents/patients and is better for business overall. This inspirational message will share the Unmistakable Before Profit and provide some of the tools you need to make a broad shift in how you approach business, whether nonprofit or for-profit. Faculty: Mike Navakoski & John Parker, Elzinga & Volkers Electronic Medical Records: Strategic Positioning Long term care, post-acute, and licensed homecare providers are facing various new regulations to the Improving Medicare Post-Acute Care Transformation (IMPACT) Act of 2014.

5 Requirements in the Act calls for enhanced reporting of standardized patient assessment data, quality measures, resource use and other measures. It also requires that this data be interoperable to allow for exchange among providers across the continuum of care. This session addresses the challenges and opportunities presented by the IMPACT Act through the implementation of HIT/HIE best practices, technology, and solutions. Faculty: Chris Ford, Great Lakes Health Connect Enhancing Your Revenue Cycle The focus of operations inside of your skilled care facility will soon be focused on data. The data produced by your clinical operations will soon be responsible for your public image, reporting of quality measures, outcomes and 5 Star rankings. Join Leah s dynamic presentation with no-nonsense advice and innovative ideas for managing reimbursement, understanding the value of documentation and the MDS.

6 Faculty: Leah Klusch, The Alliance Training Center Is Your Not-For-Profit Status at Risk In June of 2017, Baruch Senior Ministries won a major victory in the Michigan Supreme Court after a local assessor challenged their Not-for-Profit status. Even with this case as precedent to the professionals making these determinations, United Methodist Retirement Communities will be headed back to the Michigan Supreme Court to appeal another misinterpretation of the law by a lower court. Come learn more about the tactics being used to question you re not-for-profit status and how the Board of LeadingAge Michigan is joining the debate. Faculty: Tom Nobel, Baruch Senior Ministries & Phyllis Adams, Dykema Gossett Measures & Strategies for Success We are continually faced with new quality measures and new payment systems along with greater expectations from the public, surveyors and policy makers.

Join us to learn how to evaluate and leverage your existing program with your staff, board, and other key leadership groups to produce better results for your community. If you are looking to begin or refresh a program this is the session for you.

Faculty: Cedric Richner, Richner + Richner Scanning the Marketplace Learn about key senior living trends from Ziegler s strategic and industry-based research. Including consumer demand, growth trends through partnerships and affiliations, home and community-based services, campus repositioning and development, technology and innovation, and the rise of workforce challenges. There will also be a high level overview of the current state of the senior living capital markets which have been buffeted by the recent cuts to the corporate tax rates. Tom will also present a finance strategies breakout session where he will provide a deep-dive into senior living capital markets updates, President Trump s new tax bill and the impact it is having on aging services organizations.

9 Faculty: Tom Meyers, Ziegler Solutions for Middle Market Senior Housing This session will include the missional perspective and strategic significance of extending housing and services to the middle market. We will profile successful middle market senior housing models and identify key success factors for development including market characteristics, campus integration, design uniqueness, pricing parameters and financing structures. Understanding the increasing pressure on non-profit providers to maintain financial performance, honor the past, maintain their long standing history and reputation in the market, and meet the demands of the emerging middle market, we will offer strategic solutions that may bring a middle market senior housing product to reality for your organization. This session will profile the latest design parameters including common space designs, accommodation styles and overall building characteristics and layouts that reduce construction costs and maximize efficiencies.

10 We will include product pricing and innovative financing scenarios that position your organization for a successful launch of a middle market senior housing product offering. Faculty: Beverly Asper, Baker Tilly; Lynn Daly, BB&T Capital Markets & Craig Kimmel, RLPS Architects Transformational Change & Collaboration This presentation revisits the notion of transformational change and collaboration for senior care housing and services providers. Last year we presented case studies addressing the potential sale or joint venture of SNF beds, bed relocations, as well as co-ownership arrangements. This year we will be taking a deeper dive into the pros and cons of external networks, safe guarding the sharing of information and suggestions on how to potentially develop your own preferred provider organization. Faculty: Phyllis Adams, Dykema Gossett Understanding Payment Reform This session will address the latest Medicare payment reform initiatives and their impact on your organization.
