Suffolk County residents/property owners must provide two forms of proof of residency, one proof must be a photo (driver s license) with a physical address. If non-Suffolk County or Box address, two additional proofs required. Additional proofs can be any of the following: Suffolk County property tax bill, property deed (dated less than 6 months), utility bill, a Suffolk County voter s registration card, vehicle registration, library card, or an official residential lease for a minimum of nine months with same address as photo Post Office boxes are not accepted as proof of residency. (*Community Emergency Response Team) volunteer must provide valid Team Member photo card with expiration date. Auxiliary Police required. Firefighter/EMT identification card with a valid expiration date, or a letter on official stationary, dated within one year of date of purchase from their fire department/ambulance company/police department.
