1 Department of Homeland Citizenship and Immigration ServicesInstructions for I-90, Applicationto Replace Permanent Resident CardOMB No. 1615-0082; Expires 06/30/09 This form is for permanent residents and conditional residentsto apply to the Citizenship and Immigration Services(USCIS) for replacement of permanent resident Is the Purpose of This Form?1. If you became a conditional resident through marriage toa citizen or permanent resident, submit Form I-751, petition to remove conditions on residence ; or2. If you became a conditional resident based on a financialinvestment in a business, submit Form I-829, Petitionby Entrepreneur to remove If you have been a lawful permanent resident in the UnitedStates and are now taking up Commuter status whileactually residing outside the United States; or5.

2 If your status has been automatically converted topermanent resident; or2. Within 30 days after reaching your 14th birthday, toreplace a card issued before your 14th birthday; orIf you are a permanent resident, you must also file thisapplication:If you are a permanent resident or conditional resident, filethis Application :4. Write your name and Alien Registration Number (A #), if any, at the top of each sheet of paperand indicate the section and number of the item to which the answer Unless specifically required that an originaldocument be filed with an Application or petition , an ordinarylegible photocopy may be submitted. 6 If you areapplying to replace a card because of a name change, youmust bring the original court order or a certified copy ofyour marriage certificate reflecting the new name whenyou appear in person at the ASC. If you arefiling this Application to replace a permanent resident card thatwas never received, you should do the following:2.

7 When completed, mail your Application to the USCIS service center or National Benefits Center (NBC) thatprocessed your Form I-551 permanent resident card orpreviously filed Form I-90 Prepare another Form I-90. On the first page of the form,under Part 2, Application type., Number 2, Reason forapplication, check box b that reads: "My authorized cardwas never received." Include any documents with yourapplication that support your find the service center or NBC mailing address, refer to theForm I-797, Notice of Action, and/or e-Filing confirmationreceipt that you received for your previously filed Form I-485(adjustment of status), I-751 (removal of conditions ) or I-90application. In addition to the new Form I-90 Application , youshould send a copy of the Form I-797 and/or e-Filingconfirmation receipt issued for your previously : Form I-90 applications with reason b checked (Myauthorized card was never received.)

8 Do not require theapplication base fee or biometrics a Card That Was Issued With Incorrect Dataon Account of a USCIS Error. If you are submitting yourapplication because of this situation, you should do thefollowing:1. Prepare another Form I-90 Application . On the first pageof the form, under Part 2., Application type., Number 2,Reason for Application , check box d that reads: "My cardwas issued with incorrect information because of a USCIS administrative error." Include any documents with yourapplication that support your : While many of our customers are eligible to e-file,there are restrictions for some applicants. Please check ourwebsite at for a list of who is eligible to e-filethis form and for Instructions on how to properly completeand submit the you choose to file this paper version of the form, you mustinclude a check or money order with the Application to pay theappropriate filing your Application , USCIS will inform you inwriting when to go to your local USCIS Application SupportCenter (ASC) for your biometrics addition, applicants are required to submit the originalForm I-551 card containing incorrect information thatsupports the requested I-90 applications submitted with reason d checked (Mycard was issued with incorrect information because of aUSCIS administrative error.)

The letter liststhe address of the service center or NBC that processed yourpreviously filed Form I-485, I-751 or I-90 Application .
