Write your name and Alien Registration Number (A #), if any, at the top of each sheet of paperand indicate the section and number of the item to which the answer Unless specifically required that an originaldocument be filed with an Application or petition , an ordinarylegible photocopy may be submitted. If you arefiling this Application to replace a permanent resident card thatwas never received, you should do the following:2.

7 When completed, mail your Application to the USCIS service center or National Benefits Center (NBC) thatprocessed your Form I-551 permanent resident card orpreviously filed Form I-90 Prepare another Form I-90. On the first page of the form,under Part 2, Application type., Number 2, Reason forapplication, check box b that reads: "My authorized cardwas never received." Include any documents with yourapplication that support your find the service center or NBC mailing address, refer to theForm I-797, Notice of Action, and/or e-Filing confirmationreceipt that you received for your previously filed Form I-485(adjustment of status), I-751 (removal of conditions ) or I-90application. In addition to the new Form I-90 Application , youshould send a copy of the Form I-797 and/or e-Filingconfirmation receipt issued for your previously : Form I-90 applications with reason b checked (Myauthorized card was never received.)

8 Do not require theapplication base fee or biometrics a Card That Was Issued With Incorrect Dataon Account of a USCIS Error. If you are submitting yourapplication because of this situation, you should do thefollowing:1. Prepare another Form I-90 Application . On the first pageof the form, under Part 2., Application type., Number 2,Reason for Application , check box d that reads: "My cardwas issued with incorrect information because of a USCIS administrative error." Include any documents with yourapplication that support your : While many of our customers are eligible to e-file,there are restrictions for some applicants.
