The link has also been updated under the Beneficiary Information heading on the Forms page of the Kansas Medical Assistance Program ( kmap ) website. It is available for immediate use by facilities. During this transition time, KDHE will continue to accept the previous version of the form through August 31, 2017. Previous versions of the form will not be accepted after this date.
