1 LES Request form This form is for active duty Army/Air Force periods prior to October 1991 and Army/Air Force Reserve/National Guard prior to October 1994. For active duty LES documents from October 1991 to present, contact your Defense Military Pay Office if you are still on active duty. If you are Reserve/National Guard presently but requesting prior active duty LES documents, contact your unit. Your unit can obtain LES documents from the Regional Support Command Finance Division. For Reserve/National Guard LES documents from October 1994 to present, contact your unit. Your unit can obtain LES documents from the Regional Support Command Finance Division.

2 In order to process your Request for LES documents, we will need ALL of the following information. If any of the information is missing, we will be unable to process your Request . We only have records for Army and Air Force at this facility. For Marine or Navy records contact: If you are not the requesting Service Member, please include a power of attorney or a signed release of information from the service member. In cases where the Service Member is deceased we will need a copy of the death certificate.
