Refer to CDC Clinical Considerations for Use of COVID-19 Vaccines for detailed guidance. The most current version of these tables are available online. IMMUNOCOMPETENT PATIENTS Vaccine Dose Patient Age 5-11 12-17 18+ Pfizer1/Comirnaty (mRNA vaccine) Primary 2-dose series2 (3-8 weeks apart3) Booster dose2,4 (at least 5 months after 2nd dose) Any vaccine; mRNA preferred Moderna5/Spikevax (mRNA vaccine) Primary 2-dose series2 (4-8 weeks apart3) Booster dose2,4 (at least 5 months after 2nd dose) Any vaccine.

MRNA preferred Johnson & Johnson (J&J)/Janssen Note: J&J vaccine is an alternative vaccine option for persons in certain situations Primary single dose2 Booster dose2,4 (at least 2 months after 2nd dose) Any vaccine; mRNA preferred Fully Vaccinated with non-FDA-authorized/ approved series6 Booster dose2,4 (at least 5 months after primary series) Only Pfizer authorized Only mRNA authorized Footnotes (1) Pfizer has two different formulations.

Persons age 12 and older receive 30 g dose in mL (purple cap, which needs diluent, or gray cap, which does not require diluent). Age 5-11: 10 g dose in mL (orange cap). (2) People with known current SARS-CoV-2 infection should defer vaccination until they have recovered from the acute illness (if symptoms were present) and they have discontinued isolation. For individuals diagnosed with MIS-C/MIS-A, refer to Clinical Considerations. (3) Timing of 2nd dose. An 8-week interval may be optimal for some people aged 12-64, especially for males 12-39 years of age. A longer interval may improve vaccine effectiveness and lower the risk of myocarditis.

The shorter 3-week (Pfizer) and 4-week (Moderna) are recommended for children age 11 and younger, adults age 65 years and older, and others who need rapid protection during times of high community transmission and/or are at risk of severe disease. (4) Booster doses. Only Pfizer is authorized as a booster in adolescents age 12-17. No booster is authorized for children 5-11. Persons ages 18 and older can choose between any of the three FDA-authorized/approved COVID-19 vaccines; however, mRNA vaccines are preferred. Moderna booster is a half dose (50 g in mL). Only the mRNA vaccines (Pfizer/Moderna) are authorized as a booster for people who did not receive an FDA-authorized/approved COVID-19 vaccine series.

(5) Moderna has two dose volumes. The full dose (100 g in mL) is authorized for the primary series. A half doce (50 g in mL) is authorized for booster doses. (6) Persons who are considered fully vaccinated with a non-FDA-authorized/approved series include those who completed a WHO-EUL COVID-19 vaccine series, those who completed a heterologous (mix and match) series composed of any combination of FDA-approved, FDA-authorized, or WHO-EUL COVID-19 vaccines, and those who have received the full series of an "active" (not placebo) COVID-19 vaccine candidate for which vaccine efficacy has been independently confirmed ( , by a data and safety monitoring board).

(3) Additional primary doses. A 3-dose primary series is recommended for mRNA vaccines. Persons who received a single J&J dose should receive an additional (2nd) mRNA vaccine dose. If Moderna COVID-19 vaccine is used for the additional dose, administer a full 100 g ( mL) dose. Only mRNA vaccines (Pfizer/Moderna) are authorized as additional doses for people who did not receive an FDA-authorized/ approved COVID-19 vaccine series. (4) Moderna has two dose volumes. The full dose (100 g in mL) is authorized for the primary series and for additional doses. A half dose (50 g in mL) is authorized for the booster dose.

(5) Booster doses. Only Pfizer is authorized as booster in adolescents age 12-17. No booster is authorized for children 5-11. Persons ages 18 and older can choose between any of the three FDA-authorized/approved COVID-19 vaccines; however, mRNA vaccines are preferred. Moderna booster is a half dose (50 g in mL), even for immunocompromised persons. Only the mRNA vaccines (Pfizer/Moderna) are authorized as a booster for people who did not receive an FDA-authorized/approved COVID-19 vaccine series.
