Serious Incident Report This rapid action revision dated 09 February 2006-- o Requires that all deaths (except for those occurring due to natural causes). that occur in Army Medical Treatment Facilities be reported to HQDA (para 2- 3m). o Modifies paragraph 2-3m for clarity (deletes the word unattended ). o Changes office of the Provost Marshal General's electronic mail address. 2 O Establishes the Provost Marshal General as the proponent for this regulation (para 1-4a). o Mandates that all Serious Incident reports for a soldier is listed as a subject state whether the soldier has deployed within the past year (para 2- 1e). o Mandates that all reports involving the death of a soldier include whether the next of kin has been notified, and that reports of traffic accident fatalities state whether seatbelts were used or whether alcohol was involved (para 2- 3m). o Changes the reporting status of reported cases of child abuse that occur within a family unit (para 2-3t). o Adds Serious domestic violence incidents as a category 2 reportable Incident (para 2-3v).
