PARTY BUS RENTAL AGREEMENT

1 PARTY BUS RENTAL return this form with your $150 refundable deposit following are rules on the bus and if broken could result in a loss of deposit. No smoking No illegal drugs No underage drinking No standing on seats In order to prevent excessively large spills, kegs and barrels of beer, or water coolers filled with liquids are prohibited Guests are welcome to bring their own alcohol beverages and enjoy them while riding the bus. However, guests may not carryopen containers of alcohol onto or off the bus. If you do spill, please clean it up right away. No guns and knives While the bus is in motion, passengers must sit or stand behind the driver's privacy curtain If you wish to bring food you may. Please, no foods that will leave permanent odors such asfish. Jello Shots are allowed but please clean up the mess No sitting on the bar or back of seats No glitter and confetti Please use the PARTY pole in a safe manner by keeping feet on the floor Standing while bus is in motion could resultin passenger injury.

2 Getaway Bus Service is not responsible for any injuries due to a passenger who stands while the bus is in motion. DEPOSIT & PAYMENT & GRATUITY A refundable deposit of $150 is required to hold your date and serve as a security deposit for any damages. Please send your deposit check and contract within 2 days of confirming that you are making a reservation. The deposit check is separate from the payment/gratuity and will be returned as long as the contract is followed. Please make your check out to Dan Miller and mail it to: Getaway Bus Service, 716 Fountain Street, Eau Claire, WI payment is due upon the time of pick-up. The full payment can be made in CASH at the time of pickup, or by CHECK or CREDIT CARD if paid at least 1 week prior to your RENTAL date. All CHECKS must be made out to Dan Miller. If paid by CREDIT CARD a credit card vendor fee will be added to the cost.

3 All prices include sales tax. A 20% voluntary driver gratuity will be added to the cost of your POLICY & FEES The driver inspects each vehicle before, during, and after each RENTAL . In the event of damage, the customer shall be responsible for any repairs or cleaning. The following will result in automatic loss of your $150 deposit: Cracked or Broken Windows Smoking Burns Damage to Seats Damage to Lights, TV, or Stereo Equipment Excessively Spilled Fluids Any Bio-hazardous Fluid Left BehindShould damage exceed the $150 deposit fee, the customer will be held responsible for the 1 of 2 CANCELLATION A cancellation notice is required at least thirty-two (32) days in advance to receive a full refund of your deposit.
