1. POSITION : Community Support Worker 2. PROGRAM/SERVICE: Aged & Disability Services 3. POSITION PURPOSE: CatholicCare is the welfare ministry of the Catholic Church in the Diocese of Wollongong. 2 They will be required to develop a professional and trusting relationship with clients by providing a reliable, high quality service in which clients' needs are of prime importance and, wherever possible, clients are the decision makers in relation to their own care. 4. 3 KEY ROLES AND RESPONSIBILITIES Program Specific Client Focus Develop a professional rapport and maintain regular communication with individuals and parents/significant others to enhance support for achieving individual goals. Provide information to individuals, parents/significant others to facilitate informed decision making in relation to support options. Maintain a person centred approach in all activities Encouraging individuals to be self advocates for their chosen services and activities (doing it with them rather then for them). Maintain and protect a individual's safety while respecting the individual's dignity of risk. Take prompt and effective action in relation to issues, concerns or suggestions raised by individuals and parents/significant others and stakeholders.

4 Planning In conjunction with Support Facilitator and relevant stakeholders participate in the monitoring, reviewing and evaluation of the individual's:- personal care, health, behaviour and self directed plans. Encourage independence and always seek positive and sustainable outcomes. Implementation Liaise with others involved in clients' care ( families, neighbors, and friends) where appropriate and with client permission. Liaise with Support Facilitator to ensure that all relevant information pertaining to clients and carers is communicated. Facilitate the delivery of best practice client focused services to achieve outcomes required by the Care Plan. Continually assess risks to ensure you keep yourself and the client safe at all times. Monitoring and Reporting Monitor clients' well-being and report and act on of any significant changes.

5 Monitor clients' needs to identify new service options/opportunities Comply with all relevant legislation, industry standards, funding body requirements and CatholicCare policies, including those for children and vulnerable adults. Resource Management Report if there is a more efficient way to execute your roster to the roster coordinator. Report if there is a more efficient and effective way to execute services to our clients to the Support Facilitators. General work effectively with the management team. Comply with CatholicCare's policies, procedures and relevant legislation including work Health and Safety, Security, Fire Safety, Workplace Behaviour policies and procedures. Comply with CatholicCare's CSW Handbook. Be flexible in your job function and perform any other reasonable duties and responsibilities which may be assigned to you, including redeployment to other departments/areas if required, in order to meet business demands.

6 Attend training sessions and meetings as and when required. Uphold the Code of Conduct, Ethics and maintain professional boundaries at all times. Maintain documentation which is relevant and factual ensuring that hand written material can be clearly read. Physical Requirements Frequent manual handling up to 16kg Frequent bending at knees and/ or hips. Kneeling High grip strength (above 15kg / 50%) Good cardiovascular fitness. Standing for long periods Ability to reach overhead Ability to reach below knee height Ability to reach forward Sitting for long periods Bilateral lift 10kg Unilateral carry 10kg Frequent pushing / pulling Capable of driving up to or greater than 1 hour 6. SELECTION CRITERIA Essential Qualification at Certificate III level or working towards in a relevant field aged care or disability, home and community care, assistant in nursing, enrolled nurse.

7 Willingness to work in the spirit of the organisation in line with its mission and values. Driver's licence; access to a reliable vehicle; smart telephone Current first aid certificate Willingness and availability to work flexible hours (may include weekends) Desirable Experience in working with frail elderly people and people with a disability and their carers.
