1 POSITION: Temporary Full Time Health Links Model adopter department : Health Links posting DATE: December 2017 Health Links was first introduced as a key commitment in the Ministry of Health and Long-Term care s (ministry) 2012 Action Plan for Health care , to transform the system through increased access to high quality integrated services for patients living with complex chronic conditions. Health Links is an intensive integrated Model of care , designed to address patient-centered goals and needs, through meaningful patient engagement and collaborative coordinated care planning. The ministry has mandated that each Local Health Integration Network (LHIN) develop and implement a Scale and Spread Plan to ensure the Health Links Model of care reaches a mature state by March 31, 2020, which includes aligning and embedding the Model within LHIN sub-regions.
2 To support the work required, the ministry has developed a maturity Model for Health Links , which includes deliverables and outcomes that fall within four domains (DOMAINS): DOMAIN 1: Real-Time Patient Identification DOMAIN 2: care Coordination DOMAIN 3: Patient-Centered care DOMAIN 4: Measurement and Continuous Performance Improvement. The Hamilton Niagara Haldimand Brant (HNHB) LHIN will develop the overall scale and spread plan in collaboration with frontline providers, with a focus on system level changes, and the adoption of the Model at the front line through coaching, mentoring and the implementation of best practices.
3 Health link Action Tables will be established within each sub-region to lead the local implementation of key deliverables. Position Summary The Health Links Model adopter for the sub-region is recognized for their clinical skill, and is responsible for locally supporting key deliverables in the HNHB LHIN s Health Links Scale and Spread Plan, by providing leadership in the education, mentoring and coaching of frontline staff in adopting the Health Links Model of care . The Model adopter will work in close collaboration with the other sub-region roles ( Health Links Sub-Region Transformation Leads, Health Links Sub-Region Administrative Supports, and other Model Adopters), both within and across sub-regions, to achieve the Scale and Spread Plan mandate.
4 One Sub-Region Model adopter will be hired for each of the HNHB LHIN s six sub-regions (Brant, Burlington, Haldimand Norfolk, Hamilton, Niagara and Niagara North West. Sub-Region Model Adopters will also support LHIN-wide work, in addition to work in other sub-regions, as required. Core Duties Responsibilities Provide leadership, education, mentoring and coaching to frontline providers, to support the scale and spread of the Health Links Model of care within the sub-region, and to build capacity at the frontline. Identify opportunities to expand the HNHB LHIN Health Links Model of care to new sectors, organizations and frontline care providers that service persons with complex chronic care needs.)
5 Establish relationships and engage in consultation with HNHB LHIN providers and other key stakeholders. Liaise with providers from various sectors within the sub-region to focus on and improve adoption of the Health Links Model of care . Identify and facilitate opportunities to integrate the Model into the delivery of programs and services, to support sustainability and spread. In partnership with providers, identify processes and best practices that can translate from one sector/organization to another. In partnership with patients/families/caregivers, identify processes, resources and best practices to enhance patient experience, education and self-management.
6 Develop sub-region strategies for supporting providers in complex case resolution and care planning. Develop standardized education, training and onboarding materials to support both new and existing Health Links providers. Act as education lead for the training and orientation of providers to updated Health link tools, and to new Information Technology systems, for the Health Links Model of care . Work collaboratively with the other Sub-Region Model Adopters to ensure consistency, and to share best practices. Qualifications Education: Post-secondary degree in a Health professional field, Health related field, or social services field.
7 Additional education and/or professional development in your area of expertise would be considered an asset. Experience: Minimum of one (1) year of demonstrated experience in developing Coordinated care Plans (CCPs) as part of the Health Links Model of care . Minimum of two years experience in a community setting; or a combination of community, hospital or public Health settings. Experience in primary care or mental Health is an asset. Demonstrated experience with needs assessments, case management, care coordination and/or discharge planning. Experience with intensive case management is considered an asset.
8 Demonstrated experience with adult education principles, coaching, mentoring, and counseling or interviewing techniques (example: Motivational Interviewing and Communication, Behavioural Activation, etc). Experience using the Client Health Related Information System (CHRIS) is considered an asset. Skills and Abilities Skills and Competencies: Excellent skills in interpersonal communication, collaboration, and consensus building, with the ability to build effective and collaborative working relationships, and a talent for engaging diverse stakeholders in new ways of thinking. Demonstrated expertise and ability to coach and mentor teams and individuals.
9 Excellent negotiation, mediation, conflict resolution and client advocacy skills. Demonstrated leadership, communication and facilitation skills that promote co-operation, teamwork and enhance the ability of management staff to deal with difficult communications. Experience with facilitating quality improvement initiatives, leading program evaluations and developing education and training materials. Supports a collaborative approach to learning, identifying areas for improvement and outside-the-box thinking that garners support from community partners to find innovative ways of supporting the system.
10 Self-directed individual with demonstrated ability to set priorities and effectively resolve challenging situations that involve clients and stakeholders. Ability to deliver under tight timelines with effective time management skills. Well-developed analytical and problem solving skills, with the ability to analyze, interpret and present data. Other Sub-Region Model Adopters will support LHIN-wide work, in addition to work in other sub-regions, as required. Must have valid driver s license and car to travel within the sub-region, and occasionally within the broader LHIN geography. PRIMARY RESPONSIBILITIES ALSO INCLUDE: Safety (patient, worker and workplace) is a BCHS Corporate Priority.
