There are a lot of doctors to pick from. You should also pick a doctor for each member of your family. Next, make an appointment with your doctor. This will give him or her a chance to get to know your health. Remember, you do not want to wait until you are sick to pick a doctor. All of your health care is provided at your doctor s office or through written referrals from your doctor. In an emergency call 9-1-1 or go to the nearest Hospital Emergency Room. Sometimes you may have a medical problem that is not really serious and your doctor s office is closed.

2 For those times you can use an Urgent Care service. Check this book or call 1-800-514-4561 for a list of offices. At Better Health, we want to get to know you, so that we may give you the best care. The doctors in this do not work for Better Health. Some providers may choose not to perform certain services based on religious or moral beliefs. Your book must be looked at to find out exact terms, coverage, limitations and exclusions. Note: The doctors and hospitals in this book are updated as of the day the book is printed. Sometimes new doctors are added to our plan or others may leave our plan. We advise that you call us or visit our website at to make sure that the doctor is in the Plan.

3 2 REFERRALS Your Primary Care Doctor is the doctor you will see first. If needed, he/she will send you to a specialist. They will work together for a treatment plan. SELF-REFERRALS The following specialty services do not need a referral from your doctor: Chiropractic Care You can pick any doctor from this section for up to 20 visits within a 12- month period if you are age 21 or older. Dermatology Care You can pick any doctor from this section for up to 5 visits within a 12-month period. Podiatry You can pick any doctor from this section for an evaluation. If you need more treatments, surgery, or tests, authorization is needed. Gynecology You can pick any doctor from this section for the annual well-woman exam.
